在医疗保健机构的推动下,美国药师参与的住院抗凝管理服务得到快速发展,形成了系统化的服务体系,取得了很好的效果。该体系促进了药师职业发展和多学科团队建设。其优秀经验和模式值得我国药师学习和借鉴。本文从人员组成、信息系统整合、服务实施流程、服务效果评估、团队成员教育和资格认证等五部分,综述美国药师参与住院抗凝管理服务系统的模式,值得同行参考。

Under the impetus of several American healthcare institutions, pharmacist-participated inpatient anticoagulation management services have been rapidly developed, which has provided a systematic method of care that shows a great deal of promise. It promotes the professional development of pharmacists and the building of a multidisciplinary team. This model provides Chinese pharmacists a framework to improve their own system of care. This article reviews the five major aspects of the American model of pharmacist-participated inpatient anticoagulation management service in order to provide references for Chinese colleagues. The five major aspects are the multidisciplinary group, integration of information system,implementation process, effect evaluation, and member education and qualification certification.
