Implementing and Optimizing a Laboratory Automation System in a Large Clinical Laboratory

摘要 目的探索建立高效的全实验室自动化系统的解决方案。方法通过重构实验室组织模式、简化优化流程、合理使用在线离心机、合理分配仪器开展的项目和进样量、开展血清指数检测、实施自动审核功能等,对全实验室自动化系统进行全面优化改进。比较自动化系统建立前后的工作量、人员配置、样本周转时间。结果重建并优化了实验室组织模式和检验流程;应用血清指数检测能够实现仪器对溶血、脂血、黄疸等异常样本的自动识别;信息系统可对78%的样本实施自动审核。与建立流水线初期相比,每日平均测试数增加1倍,而工作人员减少50%,平均样本周转时间(TAT)缩短70%。结论全实验室自动化系统的建设是一个复杂的过程,要充分发挥流水线系统的高效能,需要对实验室的多个流程节点进行再造和改进。 Objective To explore the solution of establishment of a high efficient total laboratory automation( TLA)system. Methods The reconstruction of organizational model,simplify of work flow,using of online centrifuge,balancing the workload among analyzers,using of automatic serum index measurement instead of human judgment,and development of auto-verification function on laboratory information system were carried out to optimize and improve total laboratory automation system. The workload,staff number and turnaround time were compared before and after this optimization process. Results A new organization model and workflow was setup in the lab. Measurement of serum index could automatically identify hemolysis,lipemia and icterus. The auto-verification program automatically verified 78% of the reports. Compared with the early establishment of the TLA system,the average daily samples were increased double after one year,while staff number reduced by 50%,and average TAT shortened by 70%. Conclusion It's a complex process to implementing a TLA system in the clinical laboratory. Reconstruction and optimization of the workflow are needed to achieve high performance of a newly setup TLA system.
