早期帕金森病隐喻理解研究

Study on metaphor comprehension in the early stage of Parkinson disease

目的了解早期帕金森病患者是否存在隐喻理解障碍,并探讨帕金森病患者隐喻理解障碍的相关因素。方法设计隐喻理解检查软件对42例帕金森病患者(PD组)及30例健康对照(对照组)进行隐喻理解研究,比较两组间隐喻理解成绩是否存在差别,以及起病侧、疾病分型对隐喻理解成绩的影响。结果帕金森病患者隐喻理解得分低于对照组〔(71.7±10.8)分vs.(81.6±7.7)分,t=4.204,P=0.000〕。病程与隐喻理解成绩呈负相关(r=-0.355,P=0.023)。右侧起病患者隐喻理解得分与左侧起病者比较〔(72.3±13.2)分vs.(70.9±7.9)分,t=0.415,P=0.681〕,以震颤为主型患者隐喻理解得分与强直少动为主型患者比较〔(72.4±10.5)分vs.(70.5±11.4)分,t=0.541,P=0.592〕差异均无统计学意义。结论帕金森病患者可能存在隐喻理解障碍,且该障碍可能随着病程的延长逐渐加重。

Objective To study the metaphor comprehension and the related influencing factors of metaphor comprehension in early Parkinson disease（PD）.Methods The metaphor comprehension software was used among 42 PD patients and 30 normal controls.The difference of metaphor comprehension score between PD patients and normal controls was compared and the effect of side of onset and subtype of disease were studied.Results The results showed that the scores significantly decreased in the PD group than those in the normal control group（71.7±10.8 vs.81.6±7.7,t=4.204,P=0.000）.Courses of the disease were negatively related to the scores of metaphor comprehension（r=-0.355,P=0.023）.The scores of right-side onset PD patients were slightly higher than those of the left-side onset PD patients,but there was no statistical difference（72.3±13.2,vs.70.9±7.9,t=0.415,P=0.681）.The scores of tremor-dominant PD patients were slightly higher than those in rigidity-dominant PD patients,but there was no statitical difference（72.4±10.5 vs.70.5±11.4,t=0.541,P=0.592）.Conclusions The metaphor comprehension impairment is observed in the early stage of PD patients.Course of the disease is negatively related to the scores of metaphor comprehension.
