三维适形放疗联合同步化疗治疗食管癌的临床疗效分析

Clinical Analysis of Three Dimensional Conformal Radiotherapy Combined with Synchronous Chemotherapy in the Treatment of Esophageal Carcinoma

目的探讨三维适形放疗联合同步化疗治疗食管癌的临床疗效。方法选择食管癌患者84例,根据随机数表法分为实验组(42例)和对照组(42例)。对照组患者采用化疗。研究组患者在对照组患者的基础上接受三维适形放疗,评价两组患者治疗后的临床疗效及生存质量。结果术后试验组临床疗效总缓解率(66.67%)明显高于对照组(40.48%),并且试验组1年内生存率(97.62%)也明显高于对照组(80.95%),差异比较有统计学意义(P<0.05);同时,试验组患者术后生存质量也明显优于对照组,差异比较有显著统计学意义(P<0.01)。治疗后,两组患者的白细胞下降、血小板下降、恶心呕吐、放射性食管炎以及血红蛋白下降等不良反应发生率比较差异无统计学意义(P>0.05)。结论三维适形放疗联合同步化疗治疗食管癌疗效显著,生存率高,生存质量好,且未见并发症发生率的明显增加,值得大力推广普及。

Objective To explore the clinical efficacy of three dimensional conformal radiotherapy combined with syn-chronous chemotherapy in the treatment of esophageal carcinoma.Methods 84 cases of patients with esophageal cancer,accord-ing to the random number table method,were divided into the experimental group （42 cases） and the control group （42 cases）. The control group were treated by chemotherapy.The experimental group received three dimensional conformal radiotherapy on the basis of the control group.Efficacy and quality of life of the 2 groups were evaluated after treatment.Results The clinical efficacy of the experimental group postoperative total remission rate （66.67%） was significantly higher than that of the control group （40.48%）,and 1-year survival rate of the experimental group （97.62%） was significantly higher than those of the control group （80.95%）,the difference was statistically significant （P〈0.05）;at the same time,survival quality of the experiment group was obviously better than that of the control group,the difference was statistically significant （P〈0.01）.After treatment,leukopenia, thrombocytopenia,nausea and vomiting,radiation esophagitis and hemoglobin decreased,there was no significant difference （ P〉0.05）.Conclusion Three dimensional conformal radiotherapy combined with concurrent radiotherapy in the treatment of esoph-ageal carcinoma has obvious curative effect,high survival rate,good quality of life,without significant increase of complications,it is worthy of popularization.
