基于网络的综合医院-社区医院合作型胃肠道肿瘤随访的应用研究

A study of the effect of general hospital-community hospital cooperative follow-up on patients with gastroenteric tumor based on internet

目的研究基于网络的综合医院-社区医院合作型胃肠道肿瘤随访的应用价值。方法 2012年6月—2013年10月,招募160例胃肠道肿瘤患者作为研究对象,随机分成实验组和对照组。实验组实施基于网络的合作型随访,对照组实施综合医院的电话随访+社区医院的社区随访。随访12个月后,评价2组患者的焦虑、抑郁、生活质量和满意度。结果随访12个月后,心理方面:实验组的焦虑和抑郁得分均较对照组低,差异均有统计学意义(P<0.01)。生活质量方面:实验组的总体健康状况和生活质量、认知功能和情绪功能等3个领域得分均较对照组高,差异均有统计学意义(P<0.05),3个症状领域和1个单一条目(失眠)得分均较对照组低,差异均有统计学意义(P<0.05)。对综合医院随访的满意度方面:实验组的交流状态、交流时间、症状控制、社会心理支持及信息支持等5个维度得分和总体得分均较对照组高,差异均有统计学意义(P<0.01)。对社区医院随访的满意度方面:实验组的满意程度较对照组高,差异有统计学意义(P<0.05)。结论基于网络的综合医院-社区医院合作型胃肠道肿瘤随访可以更好地进行症状控制、进行信息和社会心理支持及更好地提高患者的生活质量,从而获得了患者更高的满意度。

Objective To study the effect of general hospital-community hospital cooperative follow-up on patients with gastroenteric tumor based on internet. Methods From June 2012 to October 2013,160 outpatients with gastroenteric tumor were recruited,and then they were randomly divided into experimentalgroupandcontrol group. The experimental group was given general hospital-community hospital cooperative follow-up based on internet,while the control group was giventelephone follow-up and community follow-up. After twelve-month follow-up,the two groups were evaluated from anxiety,depression,quality of life and satisfaction. Results After twelve-month follow-up,the anxiety and depression of the experimental group were lighter than those of the control group（ P〈0. 01）. The scores of global health status / quality of life,cognitive function and emotional function of the experimental group were higher than those of the control group（ P〈0. 05）,while the scores of fatigue,nausea and vomiting,pain and insomnia were lower than those of the control group（ P〈0. 05）. In the follow-up of general hospital,the satisfaction with communication state,communication time,symptom management,psychosocial supportand information supportand the general satisfaction of the experimental group was higher than that of the control group（ P〈0. 01）. In the follow-up of community hospital,the satisfaction of the experimental group was higher than that of the control group（ P〈0. 05）. Conclusion For outpatients with gastroenteric tumor,general hospital-community hospital cooperative follow-up based on internet has better impact on symptom control,information support and psychosocial support,and is more highly appreciated.
