胃肠外科住院患者肠外营养使用情况分析 被引量:4

摘要 目的了解胃肠外科住院患者肠外营养(PN)使用情况,分析其使用的合理性。方法对2013年7月~2013年9月胃肠外科住院患者的肠外营养液处方进行分析。结果 285例胃肠外科住院患者接受肠外营养液共2 073份,其中热氮比值为100~300的处方1 628张,糖脂比值为1~2.4的处方1 517张,分别占总处方数的78.5%和73.2%,液体量基本≥1 500 m L,能量在1 000~2 467 kcal之间。结论胃肠外科住院患者的肠外营养液处方设计基本合理,部分处方有能量、氮量不足、糖脂用量倒置的情况。临床医师和审方药师应当加强对胃肠外营养液处方的设计和审核,根据患者的年龄、身高、体重、疾病特点、代谢情况等,制定个体化肠外营养方案,以改善患者的营养状况。 Objective To investigate the utilization of parenteral nutrition (PN) and analysis nutrition medicine use rationality. Methods From July to September in 2013, the parenteral nutrition prescription of gastrointestinal surgical inpatients were analyzed. Results 285 patients received a total of 2 073 times treatment of parenteral nutrition, 1 628 prescriptions of the ratio of calories and nitrogen between 100 to 300, 1 517 prescriptions of the ratio of sugar and fat between 100 to 300, and the total number of prescriptions were 78.5% and 73.2%, the basic amount of liquid was greater than 1500 mL, the energy between 1500 to 2467 kcal. Conclusion The parenteral nutrition formulation of gastrointestinal and pancreatic surgical inpatients design is reasonable. There is a small number of prescription the ratio of sugar and fat unreasonable, energy or nitrogen deficiency phenomenon. Clinicians and pharmacist should intensify the design of parenteral nutrition prescriptions, according to patient age, height and weight, disease characteristics, metabolism, etc, develop individualized parenteral nutrition programs and improve the nutritional status of patients.
