镇静安神汤联合米氮平治疗焦虑抑郁状态伴难治性失眠的疗效观察

Effect of zhenjinganshen decotion combined with mitrazapine in treatment of anxiety and depressive with intractable insomnia

目的观察镇静安神汤联合米氮平治疗焦虑抑郁状态伴难治性失眠的疗效。方法将80例焦虑抑郁状态伴难治性失眠患者随机分为治疗组和对照组各40例。治疗组患者采用米氮平联合镇静安神汤治疗,对照组患者采用米氮平治疗,疗程均为8周。观察2组临床疗效,于入组前、第4周末和第8周末采用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)、匹兹堡睡眠质量评分(PSQI)对2组的治疗进行评分。结果治疗组总有效率为55.0%高于对照组的32.5%,2组治疗后HAMD评分及HAMA评分均低于治疗前,且治疗组低于对照组;治疗组治疗后的睡眠质量、睡眠效率及日间功能低于治疗前,对照组治疗后的日间功能低于治疗前,差异均有统计学意义(P<0.05)。结论镇静安神汤联合米氮平治疗焦虑抑郁状态伴难治性失眠优于单纯米氮平治疗。

Objective To investigate the effect of zhenjinganshen decotion combined with mitrazapine in treatment of anxiety and depressive with intractable insomnia. Methods 80 patients of anxiety and depressive patients with intractable insomnia were randomly divided into treatment group and control group,each of 40 cases. Control group treated with mitrazapine;Treatment group treated with zhenjinganshen decotion combined with mitrazapine,for 8 weeks. Observed the clinical efficacy of two groups. The Hamilton Depression（ HAMD） scales,Hamilton Anxiety（ HAMA） scales and Pittsburgh Sleep Quality（ PSQI）scores of two groups were observed before and 4 weeks,8 weeks after treatment. Results The total effective rate of treatment group was 55. 0% higher than 32. 5% of control group; After treatment,the HAMD score and HAMD score of two groups were lower than before treatment,and treatment group were lower than control group; After treatment the sleep quality,sleep efficiency and daytime function of treatment group were lower than before treatment,After treatment the day function of control group was lower than before treatment,the differences were statistically significant（ P〈0. 05）. Conclusion The effect of zhenjinganshen decotion combined with mitrazapine in treatment of anxiety and depressive with insomnia is better than mirtazapine.
