续骨活血汤治疗肾虚血瘀型老年性桡骨远端骨折临床观察

Clinical Efficacy of Xugu Huoxue Tang for Senile Distal Radius Fractures with the Syndrome of Kidney Deficiency and Blood Stasis

目的:观察续骨活血汤治疗肾虚血瘀型老年性桡骨远端骨折患者的临床疗效。方法:纳入104例老年性桡骨远端骨折肾虚血瘀型患者,按照随机数字表法将患者分为观察组与对照组各52例,对照组给予常规的骨折复位及固定治疗,观察组在对照组治疗基础上给予续骨活血汤治疗,共服药4周,在复位当天、第2、4、6、8周5个时间点通过影像学检查参照腕关节损伤(RUSS)评分标准对桡骨远端骨折的愈合情况进行评分,在骨折愈合之后采用Gartland-Werley、PRWE的评分标准对2组患者腕关节的功能恢复情况进行评价。结果:观察组的总有效率高于对照组,差异有统计学意义(P<0.05)。观察组第2、4、6、8周的影像学RUSS评分均低于对照组,差异均有统计学意义(P<0.01,P<0.05)。对照组骨折愈合时间为(5.84±1.56)周,观察组骨折愈合时间为(5.10±1.05)周,2组比较,差异有统计学意义(P<0.05)。观察组Gartland-Werley、PRWE评分均低于对照组,差异均有统计学意义(P<0.05)。结论:服用续骨活血汤可以缩短骨折愈合的时间,促进骨折的愈合,并改善骨折愈合之后的腕关节功能,提高患者骨折愈合之后的生活质量。

Objective： To observe the effect of Xugu Huoxue tang for senile distal radius fractures with the Chinese Medicine （CM） syndrome of kidney deficiency and blood stasis. Methods： A total of 104 cases of distal radial fractures in elderly patients with the syndrome of kidney deficiency and blood stasis, in accordance with the random number table , were divided into the observation group and the control group, 52 cases in each group. The control group received conventional fracture reduction and fixation, and the observation group was treated with Xugu Huoxue tang on the basis of the treatment of the control group, all for four weeks. The five time points of the 1st day, and the 2nd, 4th, 6th, 8th weeks after surgery, imaging detecte referencing RUSS score was employed to evaluate the healing situation of distal radius fracture. And after fracture healing, Gartland-Werley and PRWE scoring criteria were applied for the two groups of patients to evaluate the recovery of wrist function. Results： Thetotal effective rate in the observation group was higher than that in the control group（P 〈 0.05）. Imaging RUSS scores in the observation group at time points of 2 nd, 4 th, 6 th week were lower than those in the control group（P 〈 0.01）. Healing time was （5.84 ± 1.56） weeks of the observation group, and was（5.10 ± 1.05） weeks of the control group, the difference being significant（P 〈 0.05）. Gartland-Werley and PRWE scores of the observation group were lower than those of the control group（P 〈 0.05）. Conclusions. Xugu Huoxue tang for senile distal radius fractures with the the syndrome of kidney deficiency and blood stasis can shorten the healing time and promote the healing of fractures, and improve wrist function after fracture healing and the quality of life.
