社区0～3岁儿童无缝式健康管理研究

Research of the seamless health management for the community children aged 0-3

目的 ：探索利用居民电子健康档案,通过信息化联动实现无缝对接,构建母婴一体化管理模式,强化儿童健康管理。方法 ：龙柏社区卫生服务中心与全区13家社区卫生服务中心和3家综合性医院及妇幼保健院信息平台联网,采用区域内保健机构共同参与的母婴一体化无缝式健康管理模式对0~3岁儿童进行管理,并比较无缝式健康管理前（2009年）与管理后（2013年）儿童健康相关评价指标的变化。结果 ：采用无缝式健康管理后,2013年儿童健康管理率、儿童系统管理率、高危儿转诊到位率,计划免疫七种疫苗接种率、合格接种率、接种及时率等数据较2009年均有相应提升,差异有统计学意义（P〈0.05）。结论 ：基于电子健康档案的孕产妇、儿童保健、计划免疫相结合的新型母婴一体化管理模式,不仅加强了社区0~3岁儿童的健康管理能力,还能更好地促进儿童健康管理趋于规范、及时、有效。

Objective： To explore the utilization of the residents electronic health records through the linkage of the information technology to achieve a seamless docking to build a mother-infant integrated information management mode to strengthen the child health management. Methods： The information platform of Longbai Community Health Service Center was linked with the information platforms including other 13 community health service centers, 3 general hospitals and maternal and child health care hospital of the whole district, and the health care institutions in the region participated the seamless integrated health management mode of the maternity and children for the 0~3 years old children, and the changes of the related evaluation index of the child health were compared before 2009 and after 2013 in the seamless health management. Results： After adopting the seamless health management mode, the child health management rate, child systematical management, high-risk child referral rate, and the inoculation rate of 7 vaccines, qualified rate and timely rate of the vaccination and other data in 2013 were improved accordingly compared with those in 2009 and the difference had the statistical significance（P0.05）. Conclusion： Based on the combination of the electronic health records of the pregnant women, child health care and immunization, the new mother-infant integrated management mode can not only strengthen the health management of the 0~3 years old children, but also promote the child health management to be standardized, timely and effective.
