冠心病患者血浆miR-20a水平与冠状动脉侧支循环形成的关系 被引量:5

摘要 目的探讨血浆miR-20a水平与冠心病患者冠状动脉侧支循环形成的关系。方法选取173例接受冠状动脉造影检查且冠状动脉狭窄度≥95%的冠心病患者作为冠心病组,根据冠状动脉侧支循环程度将患者分为侧支良好(n=92)及侧支不良(n=81)两个水平,选择同期健康者60例作为对照组。冠心病组于冠状动脉造影术前、对照组于查体当天取晨起肘静脉血,采用实时荧光定量PCR法检测血浆miR-20a,采用酶联免疫吸附试验(ELISA法)检测血清VEGF水平。采用Logistic回归分析法分析冠状动脉侧支循环形成的影响因素。以冠状动脉造影结果为金标准,评价miR-20a对冠心病患者侧支循环形成的预测效能。结果冠心病组血浆miR-20a及VEGF水平均低于对照组(P均〈0.05),冠心病患者中侧支良好者的miR-20a表达量及VEGF水平均高于侧支不良者(P均〈0.05)。Gensini积分(OR=0.672,95%CI为0.215-0.916,P〈0.05)、miR-20a(OR=2.304,95%CI为1.573-3.635,P〈0.01)及VEGF水平(OR=1.351,95%CI为1.142-2.951,P〈0.05)是冠心病患者冠状动脉侧支循环形成的影响因素。miR-20a判定冠心病患者侧支循环形成的灵敏性为83.7%、特异性为96.3%。结论冠心病患者血浆miR-20a水平降低,且冠状动脉侧支循环良好者高于侧支循环不良者。血浆miR-20a水平是冠状动脉侧支循环形成的影响因素。
