摘要 目的:探讨联合检测胸水中葡萄糖转运蛋白1(GLUT1)和紧密连接蛋白-4(Claudin-4)对转移性肺腺癌细胞和反应性增生间皮细胞的鉴别诊断价值。方法:应用免疫组化法检测GLUT1和Claudin-4在30例肺腺癌、25例反应性增生间皮细胞胸水中的表达情况,并分析二者单独及联合检测对肺腺癌诊断的灵敏度及特异性。结果:肺腺癌细胞GLUT1和Claudin-4呈阳性,反应性增生间皮细胞GLUT1和Claudin-4呈阴性。两种抗体联合检测对肺腺癌诊断的灵敏度和特异性分别为100%和92.0%。结论:联合检测胸水GLUT1和Claudin-4对转移性肺腺癌细胞和反应性增生间皮细胞的诊断及鉴别诊断具有重要的临床应用价值。 Objective: To assess the diagnostic value by combined detecting the glucose transporter protein 1( GLUT1) and the tight junction protein( claudin-4) in the pleural effusion in patients with metastatic adenocarcionoma of lung and reactive mesothelial cells. Methods: Immunohistochemistry was used to detect the expression of GLUT1 and claudin-4 in pleural effusion in 30 patients with adenocarcinoma of lung and 25 with reactive mesothelial cells,and the sensitivity and specificity were compared between combined evaluation and single detection. Results: Both GLUT1 and claudin-4 were positive in the pleural effusion of lung cancer patients and negative in reactive mesothelial cells. The sensitivity and specificity by combined detection of GLUT1 and claudin-4 were 100% and 92. 0% in metastatic adenocarcinoma of lung. Conclusion: Combined detection of GLUT1 and claudin-4 is of important value in the diagnosis and differential diagnosis of adenocarcinoma cells and reactive mesothelial cells in the pleural effusion.
