开腹与腹腔镜治疗卵巢子宫内膜异位囊肿合并不孕症的临床比较

Clinical Comparison of the Effect Between Open and Laparoscopic Operation in the Treatment of Ovarian Endometriosis Cyst Complicated with Infertility

目的探讨开腹与腹腔镜治疗卵巢子宫内膜异位囊肿合并不孕症的治疗效果。方法随机选取100例来该院2011年1月-2014年1月间诊治的卵巢子宫内膜异位囊肿合并不孕症患者,按照随机分组法将其分为观察组与对照组各50例,观察组患者实施腹腔镜治疗,对照组患者进行开腹手术治疗,比较观察两组病人的临床治疗效果。结果观察组患者的治疗总有效率为94.00%,对照组患者的治疗总有效率为72.00%,前者明显后者,差异有统计学意义(P<0.05);两组在手术时间、术中出血量以及肛门排气时间和住院时间方面对比,观察组均少于对照组,差异有统计学意义(P<0.05);在术后复发率、成功受孕率及患者满意度方面,观察组分别为2.00%、44.00%及98.00%,均优于对照组的12.00%、20.00%及80.00%,差异有统计学意义(P<0.05)。结论卵巢子宫内膜异位囊肿合并不孕症的患者,通过实施腹腔镜手术治疗可以减少手术与住院时间、肛门排气时间及术中出血量,同时还能降低术后复发率,使受孕率提高,患者满意度增加,可以在临床进一步推广使用。

Objective To study the effect of open and laparoscopic operation in the treatment of ovarian endometriosis cyst complicated with infertility.Methods 100 patients with ovarian endometriosis cyst complicated with infertility diagnosed and treated in our hospital from January 2011 to January 2014 were selected and randomly divided into the observation group and control group with 50 cases in each.The observation group were given laparoscopic operation for treatment,and the control group were given open operation.And the clinical treatment effect was observed and compared between the two groups.Results The overall response rate of the observation group was 94.00%,much higher than 72.00%of the control group with a statistically significant difference（P〈0.05）;compared with the control group,the duration of operation,intraoperative blood loss and anal exhaust time and hospital stay of the observation group was less respectively,with statistically significant difference（P〈0.05）;compared with the control group,the observation group had lower postoperative recurrence rate（2.00%versus 12.00%）,higher successful pregnancy rate（44.00%versus 20.00%） and higher patient satisfaction（98.00%versus 80.00%） with statistically significant differences（P〈0.05）.Conclusion For patients with ovarian endometriosis cyst complicated with infertility,laparoscopic operation treatment can reduce the duration of operation and length of stay,anal exhaust time,intraoperative blood loss and postoperative recurrence rate as well as improve the rate of pregnancy,so it is worthy of further application and promotion in clinical practice.
