放疗联合替莫唑胺治疗老年人恶性胶质瘤的护理研究

Nursing experience in the treatment of elderly patients with malignant gliomas treated with radiotherapy combined temozolomide chemotherapy

目的总结对老年恶性脑胶质瘤术后患者行常规放疗联合口服化疗药物替莫唑胺(TMZ)的护理体会。方法收集2000年1月至2014年1月就诊于我科的23例患恶性胶质瘤老年患者,在采用放疗联合替莫唑胺化疗的同时,针对所有患者施行有效的护理。结果 23例同步放化疗患者中,出现恶心、呕吐者6例,便秘者5例,乏力者1例,Ⅱ度骨髓抑制者1例,主要表现为白细胞计数下降,给予对症治疗后白细胞恢复正常,未延误放化疗的正常进行。所有患者均未出现重度骨髓抑制及癫疒间发作,放疗后23例患者均出现局部脱发。结论针对老年恶性脑胶质瘤术后患者,在同步放化疗的过程中,施行有效的个体化护理手段,可以明显降低老年患者的不良反应,对治疗有积极作用。

Objective To summarize the nursing experience of conventional radiotherapy combined with oral chemotherapy drug temozolomide（ TMZ） in the treatment of elderly patients with malignant glioma after surgery.Methods Twenty three cases of elderly malignant glioma patients who were treated in our department from January2000 to January 2014 were collected. The patients received radiotherapy combined temozolomide chemotherapy as well as effective nursing. Results During the treatment,there were 6 cases of nausea and vomiting,5 cases of constipation,1 case of weakness,and 1 case of Ⅱ myelosuppression which was reflected mainly by the decrease of white blood cells number. The white blood cells number returned to normal after the symptomatic treatment without delaying the normal radiation and chemotherapy. No severe bone marrowsuppression or seizures was observed,but all the patients had local hair loss after radiotherapy. Conclusion The implementation of effective personal care measure can significantly reduce the adverse reactions of elderly patients with malignant brain glioma after surgery and also play an active role in the process of synchronous radiotherapy and chemotherapy.
