美国最新高危型人乳头状瘤病毒检测用于宫颈癌初筛指南介绍 被引量:2

摘要 继2011年ACS(American Cancer Society,ACS)、ASCCP(American Society of Colposcopy and Cervica I Pathology,ASCCP)、ASCP(American Society of Clinical Pathology,ASCP)对宫颈癌及癌前病变筛查指南公布后,随着几项大型临床研究数据的发表,支持hr HPV检测用于初筛的证据越来越多。特别是在美国本土的一项前瞻性研究"满足高级别HPV诊断需求(Addressing the Need for Advanced HPV Diagnostics,ATHENA)"试验,揭示hr HPV检测用于初筛在单轮筛查中对于CIN2、CIN3的敏感性优于单独细胞学筛查,并明确提出对hr HPV阳性女性分流的策略。本文旨在对美国最新hr HPV检测用于宫颈癌初筛指南的新建议及相关证据进行介绍,同时提出对过渡期指南临床应用时带来的问题包括hr HPV取样的质量控制以及可能存在不同于HPV致癌的其他肿瘤发生途径等临床研究的必要性。
