内镜下黏膜剥离术治疗早期食管癌及癌前病变的近期疗效和安全性

Short-term efficacy and safety of endoscopic submucosal dissection in the treatment of early esophageal cancer and precancerous lesion

目的评价内镜下黏膜剥离术(endoscopic submucosal dissection,ESD)治疗早期食管癌及癌前病变的近期疗效和安全性。方法选取本院100例早期食管癌或癌前病变患者进行ESD治疗,评估ESD治疗的近期疗效和安全性。结果所有患者均顺利完成ESD,一次性剥离病变成功率为93%,患者平均手术时间为(73.4±8.5)分钟,术后发生胸骨后疼痛45例、穿孔3例、迟发性出血1例、腹痛10例、进食梗阻8例,复发2例,再次行ESD。结论 ESD能够安全有效地治疗早期食管癌及癌前病变,值得临床应用。

Objective To evaluate the short-term efficacy and safety of endoscopic submucosal dissection（ESD） in the treatment of early esophageal carcinoma and precancerous lesions.Method 100 patients with early esophageal carcinoma or precancerous lesions were recevied ESD therapy,evaluated the shortterm efficacy and safety of ESD.Result All patients were successfully completed ESD therapy,one-time dissection success rate was 93%,the mean operation time was（73.4±8.5） minutes;after ESD,45 cases of postoperative sternal pain,3 cases of perforation,1 case of delayed hemorrhage,10 cases of abdominal pain and 8 cases of dysphagia;2 cases recurred again to need second ESD.Conclusion ESD is safe and effective in the treatment of early esophageal cancer and precancerous lesions,it is worthy of clinical application.
