腹膜淋巴回流途径在腹腔感染早期内毒素肺损伤中的作用 被引量:2

摘要 目的探讨腹膜淋巴回流途径在腹腔感染早期内毒素肺损伤中的作用。方法 Wister大鼠72只分A、B、C 3组:A组开腹后翻动盲肠注射生理盐水,B组开腹后行盲肠结扎穿孔手术,C组开腹后结扎胸导管,再行盲肠结扎穿孔手术。前两组分别于术后3、6、12、24 h检测大鼠胸导管淋巴浆、门静脉血浆及主动脉血浆内毒素浓度;再检测3组肺组织中NF-κB P65浓度,并观察肺组织病理,计算湿/干重比(W/D)及检测细胞凋亡,评估肺损伤程度。结果 B组相同时相点胸导管淋巴浆内毒素的浓度高于门静脉血浆(P<0.01)和动脉血浆(P<0.05),而动脉血浆高于门静脉血浆(P<0.05);B组不同时相点门静脉血浆内毒素的含量差异无统计学意义(P>0.05);B组同一时相点,淋巴浆、门静脉血浆、主动脉血浆中内毒素的浓度均高于A组(P<0.05);B组肺组织NF-κB P65的含量、病理HE评分、W/D、凋亡指数均高于相同时相点的A组(P<0.05);同一时相点C组肺组织NF-κB P65的含量、病理HE评分、W/D、AI均低于B组(P<0.05),但高于A组(P<0.05)。结论腹腔感染早期,内毒素的腹膜淋巴回流途径在肺损伤中发挥重要作用,而阻断腹膜淋巴回流途径可以抑制内毒素的淋巴回流,从而减轻其所致的肺损伤。
