胃超声造影对胃部疾病诊断价值的研究

The research of diagnose value for gastric diseases by gastric contrast-enhanced ultrasonography

目的探讨胃超声造影对胃部疾病诊断价值情况。方法选择延安市人民医院2013年5月～2015年5月收治的300例疑似胃部疾病患者的临床资料，所有患者均通过超声造影检查、常规超声检查，胃癌患者进行术后组织病理学确诊。观察胃超声造影和常规超声对胃部疾病诊断腈况．观察胃癌超声造影TNM分期和病理结果比较情况。结果胃超声造影和常规超声诊断急性胃炎敏感性为61．5％、53．9％，特异性为93．0％、86．1％；胃溃疡敏感性为90．5％、87．0％，特异性为78．O％、74．0％；胃平滑肌瘤敏感性为71．4％、57．4％，特异性为91．4％、86．3％；两种方法比较差异无统计学意义（P〉0．05）。胃超声造影诊断胃癌敏感性为90．0％、特异性为94．5％均高于常规超声（43．8％、79．5％），差异有统计学意义（P〈0．05）。胃癌超声造影T分期准确率为70．0％、N分期准确率为76．3％、M分期准确率为92．5％。结论胃超声造影对胃部疾病诊断均有较高的价值，尤其是在胃癌的晚期诊断准确率较高，值得临床推广应用。

Objective To approach diagnose value for gastric diseases by gastric contrast-enhanced uhrasonography. Methods From May 2013 to May 2015, in Yan＇an People＇s Hospital, the clinical data of 300 patients with suspecting gastric diseases were selected, all of them got gastric contrast-enhanced uhrasonography examination, conventional ultrasound examination, and patients with gastric cancer were definite diagnosis by histopathology. The diagnose for gastrie diseases by gastric contrast-enhanced ultrasonography examination and conventional ultrasound examination were detected, the comparison of TNM stages of contrast-enhanced ultrasonography and pathologic result were detected. Results The sensitivity of gastric contrast-enhanced uhrasonography examination and conventional ultrasound examination diagnosing acute gastritis were 61.5% and 53.9%, specificity were 93.0% and 86.1%; sensitivity of diagnosing gastric ulcer were 90.5% and 87.0%, specificity were 78.0% and 74.0%; sensitivity of diagnosing gastric leiomyoma were 71.4% and 57.4%, specificity were 91.4% and 86.3%, two methods were compared, the differences were not statistically significant （P 〉 0.05）. The sensitivity of gastric contrast-enhanced uhrasonography examination diagnosing gastric cancer was 90.0%, specificity was 94.5%, these were higher than those of conventional ultrasound examination （43.8%, 79.5%）, the differences were statistically significant （P 〈 0.05）. The accuracy rate of gastric contrast-enhanced ultrasonography examination diagnosing gastric cancer T stage was 70.0%, N stage was 76.3%, M stage was 92.5%. Conclusion The diagnose for gastric diseases by gastric contrast-enhanced uttrasonography examination has high value, above all accuracy rate of diagnosis is high in the late stage of gastric cancer, which is to be used.
