京津唐地区1583例老年股骨转子间骨折流行病学特征分析

Epidemiological characteristic analysis of 1583 cases of elderly patients with femoral intertrochanteric fractures in Beijing, Tianjin and Tangshan

目的：分析京津唐地区老年股骨转子间骨折的流行病学特征。方法收集京津唐地区8家医院自2010年1月至2011年12月间就诊的骨折患者的数字化影像资料和基本资料进行回顾性分析，筛选出60岁以上的股骨转子间骨折患者，记录并比较其性别、年龄、AO及Evans分型等数据，分析2年间京津唐地区老年股骨转子间骨折的流行病学特征。结果共收集老年股骨转子间骨折患者1583例，其中北京地区491例（31.0%）、天津地区293例（18.5%）、唐山地区799例（50.5%）。男性643例（40.6%）、女性940例（59.4%），男女比例为1∶1.46。左侧比右侧稍多（825/758）。平均年龄为（77.6±7.6）岁，其中71～80岁所占比例最高（45.5%）。AO分型中31-A2型所占比例最高（61.1%），31-A3型所占比例最低（11.5%）。Evans分型中Ⅰ型所占比例最高（27.4%），Ⅴ型所占比例最低（11.5%）。结论京津唐地区老年转子间骨折占各年龄段转子间骨折的73.4%，占老年髋部骨折的51.9%，女性多于男性，其中71～80岁所占比例最高，31-A2型与EvansⅠ型骨折所占比例最高。

Objective To investigate epidemiological characteristics of femoral intertrochanteric fractures in elderly patients in Beijing, Tianjin and Tangshan. Methods The digital radiography image data and basic information of patients above 60 years with intertrochanteric fractures from 2010 to 2011 were analyzed retrospectively in 8 hospitals of Beijing, Tianjin and Tangshan. All the data including patients’ gender, age, AO and Evans types were abstracted and analyzed to investigate the epidemiologic characteristics of intertrochanteric fracture occurring during this time in the districts. Results A total of 1 583 intertrochanteric fracture patients with age above 60 years old were included, 491 cases from Beijing （31.0%）, 293 cases （18.5%） from Tianjin, 799 cases from Tangshan （50.5%）, 643 （40.6%） males, 940 （59.4%） females （M：F=1：1.46）. Fractures involving left side were slightly more than the right （825/758）. The fracture was most common in age group of 71-80 years （45.5%） with the mean age was （77.6±7.6） years. By AO classification, type 31-A2 was the most common fracture type （61.1%）, with type 31-A3 as the least common type （11.5%）. By Evans classification, typeⅠwas the most common type （27.4%）, and typeⅤis the least common type （11.5%）. Conclusion In elderly patients, intertrochanteric fractures is 73.4%of intertrochanteric fractures of all ages, and 51.9%of hip fractures, female are more prone to intertrochanteric fractures than male, and the most common age is 71-80 years. 31-A2 and EvansⅠis the most common fractures type for AO and Evans classification systems, respectively.
