2014年患者感染H7N9实验室指标动态分析

Dynamic analysis of laboratory indexes of H7N9 infections in 2014

目的动态观察两例H7N9感染患者实验室指标的变化,探讨其变化特点及评估在H7N9病情发展中的价值。方法 2014年1月医院收治两例H7N9患者,住院期间采集患者外周静脉血进行常规生化、血常规、凝血功能、动脉血气及其他感染指标的动态检测,采用EXCELL进行数据分析。结果两例H7N9患者外周血白细胞计数和中性粒细胞呈升高趋势,经治疗转为正常;淋巴细胞入院时均显著降低,随病情好转而升至正常;两例患者入院时PaO2和PaCO2低于正常水平,随着氧疗逐步恢复正常,动脉血气中游离钙离子一直低于正常水平。结论上述试验指标是反映H7N9患者病情严重程度的重要指标,对其动态观察对H7N9患者的病情发展、疗效观察及预后判断等方面具有非常重要的指导价值。

OBJECTIVE To observe the dynamic changes of laboratory indexes in two patients suffered from H7N9, investigate the characteristics of the changes and assess the value of the indexes in disease development. METHODS The biochemical routine test, blood routine, coagulation functions, arterial blood gas as well as other infection markers in peripheral venous blood were detected during disease course of two cases admitted in the hos- pital in Jan. 2014. The software EXCELL was used for data analysis. RESULTS The counts of white blood cell （WBC） and neutrophils （NE%） of H7N9 patients were increased and returned to normal after treatment. Lym- phocytes （LY%） notably reduced on admission,and increased with condition improvement of patients. The levels of PaO2 and PaCO2 were below the normal level,and returned to normal step by step with oxygen therapy. The level of free CA2＋ in artery blood gas was lower than normal through the treatment. CONCLUSION The markers above may be important laboratory indexes of H7N9 patients whose conditions are severe, and the dynamic obser- vation of these makers have very vital guidance value for disease development, efficacy observation and prognosis judgment Of H7N9 patients.
