单味中药及有效成分对类风湿关节炎炎性细胞因子TNF-ɑ的影响

The Effect of Effective Components in Single Herbs on TNF-α of Rheumatoid Arthritis

类风湿关节炎(Rheumatoid Arthritis,RA)是免疫介导的炎症性疾病,其基本病理变化是自身免疫异常引起的致炎细胞因子和炎症介质等的释放,以及由此所致的关节滑膜的慢性炎症,血管翳形成,软骨和软骨下骨破坏。肿瘤坏死因子α(Tumor Necrosis Factor alpha,TNF-α)是RA病程中最早产生的细胞因子之一,在RA发病机制中起主导作用,是炎症瀑布的上游分子,通过作用于多种细胞在RA的滑膜炎症、血管新生及骨质破坏中起重要作用。因此抑制TNF-α的作用对控制RA病情及改善预后具有十分重要的作用。TNF-α拮抗剂可特异性的拮抗TNF-α,抑制RA的病情进展。目前临床上已使用的TNF-α拮抗剂主要有依那西普、英夫利西单抗和阿达木单抗等,具有较好的临床疗效,但因其价格昂贵且可能诱发诸如感染、肿瘤、自身免疫病等不良反应,限制了其广泛应用。中医中药具有明显的TNF-α拮抗作用,临床具有一定疗效,且不良反应较少,价格相对合理,具有良好的应用前景。该文就近年来单味中药及其有效成分对类风湿关节炎炎性细胞因子TNF-α的影响作一综述。

Rheumatoid Arthritis（RA）is an immune-mediated inflammatory diseases. The basic pathological change is the release of inflammatory cytokines and inflammatory medium resulted from abnormal autoimmune,and that causes chronic synovitis of joints,pannus formation,destruction of cartilage and subchondral bone. Tumor Necrosis Factor alpha（TNF-α）is one of the earliest cytokines produced in the course of RA and plays an important role in the mechanism of this disease. TNF-α is an upstream molecule of inflammation waterfall,palying a leading role in induces synovial inflammation,angiogenesis and bone destruction by various cells in RA. For the diseases control and improve its survival rate of RA,it is necessary to inhibit the releasing of TNF-α. TNF-α antagonists can slow the deterioration of RA through specifically inhibit TNF alpha. Presently,those TNF alpha blockers such as Etanercept,Infliximab and Adalimumab resistance has been used clinically and proved has certain clinical effect,but the expensive costs and it＇s adverse reactions such as infection,tumor,autoimmune disease limits the application of those medicine. Chinese medicine has obvious antagonistic action on TNF alpha,it＇s has been clinical proved effective and few adverse reaction,for another,the price is relatively reasonable,so it＇s deserves to clinical application. This article summarized the unit herbs and its effective components on the influence of inflammatory cytokine TNF-α in rheumatoid arthritis（RA）.
