Safety and Efficacy of Misoprostol Versus Oxytocin for the Prevention of Postpartum Hemorrhage

摘要目的比较缩宫素和米索前列醇在预防产后出血中的安全性和有效性。方法在一个随机双盲对照试验中,396位经阴道分娩的孕妇被分为两组:一组将20IU缩宫素加入1000m L林格氏液输注并口服2片安慰剂,另一组口服400μg米索前列醇(2片)并将2m L生理盐水加入1000m L林格氏液输注。结果与米索前列醇组相比,缩宫素组失血量更多。两组间在血细胞比容和血红蛋白降低方面没有显著差异。结论预防使用米索前列醇对减少产后出血量更有效,当缩宫素无效时更应使用米索前列醇。 OBJECTIVE To compare the safety and efficacy of oxytocin and misoprostol when used in the prevention of PPH. METHODS In a double-blind randomized controlled trial,396 pregnant women who had a vaginal delivery were assigned into two groups ：to receive either 20 IU of oxytocin in 1000mL Ringer＇s solution and two placebo tablets or 400 ug oral misoprostol （ as two tablets ） and 2mL normal saline in 1000mL Ringer＇ s solution. RESULTS The quantity of blood loss was higher in the oxytocin group in comparison to the misoprostol group. There was no significant difference in the decrease in hematocrit and hemoglobin between the two groups. CONCLUSION Misoprostol is more effective in preventing postpartum hemorrhage, and more useful when oxytocin is unavailable.
