摘要
目的通过对兰州地区1 880例健康人静脉血细胞参数的调查研究,建立该地区静脉血细胞各项参数的参考范围。方法采用回顾分析法,用Sysmex XE-5000全自动血细胞分析仪进行血细胞26项参数分析,观察参考值的变化。结果各项参数中,除血小板(PLT)、血红蛋白(Hb)等个别参数接近正态分布,其余均为偏态分布;白细胞6项参数中除中性粒细胞(N)百分比和淋巴细胞(L)百分比外,其余各项参数男女95%置信区间(CI)差异有统计学意义(P<0.05),红细胞8项参数中除平均红细胞血红蛋白浓度(MCHC)、红细胞体积分布宽度标准差(RDWSD)外,其余各项参数男女95%CI差异有统计学意义(P<0.05);PLT相关参数及幼稚细胞百分比男女95%CI差异均无统计学意义(P>0.05);多项参数的95%CI与目前沿用的参考范围比较差异较大。结论 95%CI与原参考范围存在较大差异,原参考范围缺乏准确性和适用性,有必要科学、合理地建立地区性的血细胞参考值范围。
Objective To establish reference ranges of venous blood cell parameters in Lanzhou area,through investigating 1 880 cases of healthy people.Methods Retrospective analysis method was adopted,and changes of 26 venous blood cell parameters were observed by using Sysmex XE-5000 automatic hematology analyzer.Results Some parameters,including platelet(PLT)and hemoglobin(Hb),were close to normal distribution,while most of parameters were skewed distribution.In the 6 parameters of white blood cells,except for percentage of lymphocyte and neutrophi,the 95%CI of the rest of parameters had statistically significant differences between male and female(P〈0.05).In the 8 parameters of red blood cell,except for mean corpuscular haemoglobin concentration(MCHC)and standard deviation of red blood cell volume distribution width(RDWSD),the 95%CIof the rest of parameters had statistically significant differences between male and female(P〈0.05).The 95%CI of PLT related parameters and percentage of juvenile cells had no statistically significant differences between male and female(P〈0.05).In some parameters,there were significant differences between 95%CI observed in this study and reference ranges currently used.Conclusion There are significant differences between 95%CI of these parameters and original reference ranges,so the original reference ranges are lack of accuracy and applicability,which indicates that it is necessary to scientificlly and rationally establish reference ranges of blood cell in region.
