摘要 目的探讨全景超声在小器官病变诊断和治疗中的作用。方法选择2011年5月至2012年5月于武装警察部队总医院就诊和治疗的小器官病变患者120例。按照3个小组进行病例选择,眼部病变组40例,乳腺病变组40例,甲状腺病变组40例,根据病情进行筛选,所有病例满足入组标准,达到病例数后停止此组的诊断。同时随机选择未发生小器官病变的患者40例作为对照组。每组患者进行全景超声,并根据结果进行病情分析和诊断划分,同时计算超声检查的灵敏度、特异度。结果眼部病变组检出病变33例,灵敏度为82.5%,特异度为92.5%;乳腺病变组检出病变39例,灵敏度为97.5%,特异度为100.0%;甲状腺病变组检出病变36例,灵敏度为90.0%,特异度为97.5%。结论全景超声在诊断眼球、乳腺、甲状腺等小器官病变上具有很高的灵敏度和特异度,同时,全景图像可以很清晰地观察到病变的位置及范围,具有很高的临床推广价值。 Objective To investigate the role of panoramic ultrasound in the diagnosis and treatment lesions in small organs. Methods A total of 120 patients diagnosed and treated in General Hospital of Chinese People's Armed Police Forces from May 2011 to May 2012 were included, and divided into three groups, ocular lesions ( n = 40) , breast lesions ( n = 40 ) , thyroid lesions ( n = 40 ). According to the disease screening, when the number of cases was reached, the diagnosis stopped. 40 patients without small organ lesions were randomly selected as control group. Each group was checked with panoramic ultrasound, and the disease condition was analyzed and diagnosis was done according to the checking results, and the sensitivity and specificity of uhrasonography were calculated. Results 33 cases of lesions were discovered in eye lesions group, the sensitivity was 82. 5 % , and the specificity was 92.5 % ;39 cases of lesions were discovered in breast lesion group, the sensitivity was 97.5 % and the specificity was 100.0 % ; 36 cases of lesions were discovered in thyroid lesion group, the sensitivity was 90.0%, and the specificity was 97.5%. Conclusion Panoramic ultrasound has a high sensitivity and specificity in the diagnosis of lesnions in small organs such as eye, breast, thyroid, which can clearly show the location and extent of the lesion, thus has a high clinical value.
