鼻唇沟皮下蒂皮瓣隧道转移法修复上唇缺损38例

Repair of upper lip defects by tunnel transfer with nasolabial fold flap of subcutaneous belt pedicle on 38 cases

目的探讨采用鼻唇沟皮下蒂皮瓣经隧道转移修复上唇缺损的方法。方法应用鼻唇沟皮下蒂皮瓣经隧道转移，对38例患者进行上唇部皮肤缺损修复，最大缺损面积3．5cm×3．0cm。结果38例患者皮瓣全部成活，其中1例因蒂部卡压术后当天皮瓣颜色略紫，术后第2天予蒂部松解后好转，1例患者转移皮瓣较臃肿，经Ⅱ期修复后效果良好；术后随访6个月到2年，皮肤质地色泽与周围皮肤相近，瘢痕不明显。结论采用鼻唇沟皮下蒂皮瓣隧道转移是修复上唇部皮肤缺损的理想方法。

Objective To evaluate the application of the method of tunnel transfer with nasolabial fold flap of subcutaneous belt pedicle to repair upper lip defects. Methods The method of tunnel transfer with nasolabial fold flap of subcutaneous belt pedicle was applied to repair the upper lip defect, which the max area was 3.5 cm × 3.0 cm. Results All the flaps were survived except for 1 case with slightly purple color on the postoperative day due to compression which was recovered by loosening the compression at 2 days postoperatively and 1 case with swelling which was better after the secondary repair. It was similar to the surrounding skin with concealed scar after 6 to 12 months following up. Conclusion It is an ideal method of tunnel transfer with nasolabial fold flap of subcutaneous belt pedicle to repair upper lip defects.
