穴位敷贴对慢性阻塞性肺疾病急性加重期患者无创通气胃肠功能紊乱的影响

Effect of Acupoint Application on Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease Non Invasive Ventilation Gastrointestinal Disorders

目的:观察穴位敷贴联合莫沙必利对慢性阻塞性肺疾病急性加重期(acute exacerbation chronic obstructive pulmonary disease,AECOPD)患者无创通气胃肠功能紊乱的影响。方法:将180例AECOPD病者随机平均分为莫沙必利组、莫沙必利加中药穴位敷贴组(穴位敷贴组)与基础治疗组,疗程均为5 d。观察3组肺部感染控制窗出现时呼吸频率、心率与血气分析结果;比较两组治疗过程中胃肠功能紊乱发生率、每天排便次数等。结果:胃肠紊乱发生率比较:莫沙必利组、穴位敷贴组显著低于基础治疗组,穴位敷贴组显著低于莫沙必利组(P<0.05);每天排便次数比较:穴位敷贴组>莫沙必利组>基础治疗组(P<0.05);各组间呼吸频率、心率与血气分析比较,差异无统计学意义(P>0.05)。结论:中药穴位敷贴联合莫沙必利能最有效改善AECOPD患者无创通气的胃肠功能紊乱症状。

Objective ： To observe the clinical effect of acupoint application combined with Mosapride on patients with acute exacerbation of chronic obstructive pulmonary disease non invasive ventilation gastrointestinal disorders. Methods： 180 cases of AECOPD patients were randomly divided into mosapride group, mosapride and Chinese medicine acupoint application group （acupoint application group） and basic treatment group. All patients were treated for 5 days. The respiratory rate, heart rate and blood gas analysis were observed in the 3 groups of pulmonary infection control window, and the incidence of gastrointestinal dysfunction and the number of defecation in the two groups were compared. Results ： Comparison of the incidence of gastrointestinal disorders ： mosapride group, acupoint application group were significantly lower than those of basic treatment group, acupoint application group was significantly lower than that of mosa- pride group （ P 〈 0.05 ） ; comparison of daily defecation ： acupoint application group 〉 mosapride group 〉 basic treatment group （ P 〈 0. 05 ） ; each group respiratory frequency, heart rate and blood gas analysis in the comparison, the difference was not statistically signifi- cant （ P 〉 0.05 ）. Conclusion ： Chinese medicine acupoint application combined with mosapride can most effectively improve the symp- toms of AECOPD patients with noninvasive ventilation gastrointestinal dysfunction.
