柔肝煎膏联合核苷类抗病毒药物对慢性乙型肝炎患者肝纤维化指标及生活质量的影响

Influence of “Rougan Decocted Extract”Combined with Nucleoside Antiviral Drugs on Liver Fibrosis Iindexes and Quality of Life for Patients with Chronic Hepatitis B

目的:观察柔肝煎膏联合核苷类抗病毒药物对慢性乙型肝炎患者肝纤维化指标和生活质量的影响。方法:纳入慢性乙型肝炎肝肾阴虚证患者66例,随机分为对照组和治疗组,每组33例。对照组采用核苷类抗病毒药物治疗,治疗组在此基础上加用柔肝煎膏治疗,疗程为24周。比较两组患者治疗前后的透明质酸(HA)、层粘连蛋白(LN)、Ⅳ型胶原(Ⅳ-C)、Ⅲ型前胶原(P-Ⅲ-P)水平,采用SF-36量表评价患者的生活质量。结果:1肝纤维化指标:治疗后,两组患者的HA、LN、Ⅳ-C、P-Ⅲ-P水平较治疗前均明显降低(P<0.05),且治疗组患者的HA、LN、Ⅳ-C、P-Ⅲ-P水平低于对照组(P<0.05)。2SF-36量表评分:治疗后,治疗组患者的躯体健康、躯体角色、总体健康、情绪角色、心理健康5个维度评分较治疗前显著升高(P<0.05),且治疗组患者的躯体健康、心理健康维度评分显著高于对照组(P<0.05)。结论:柔肝煎膏联合核苷类抗病毒药物能明显降低慢性乙型肝炎患者的血清肝纤维化指标,改善患者的生活质量。

Objective： To observe the influence of＂Rougan Decocted Extract＂combined with nucleoside antiviral drugs on the liver fibrosis indexes and the quality of life in patients with chronic hepatitis B. Methods： A total of 66 patients with chronic hepatitis B of yin deficiency of liver and kidney type were selected and randomly divided into the control group and treatment group with 33 cases in each group. The patients in the control group were treated with nucleoside antiviral drugs and the patients in the treatment group were treated with＂Rougan Decocted Extract＂combined with nucleoside antiviral drugs,with a course of 24 weeks. The levels of hyaluronic acid（ HA）,laminin（ LN）,type Ⅳ collagen（ Ⅳ-C） and procollagen-Ⅲ-peptide（ P-Ⅲ-P） were detected and the quality of life of the patients were evaluated by SF-36 scale. Results： 1Liver fibrosis indexes： After treatment,the levels of HA,LN,Ⅳ-C and P-Ⅲ-P were significantly decreased（ P〈 0. 05）,and the levels of HA,LN,Ⅳ-C and P-Ⅲ-P of the treatment group were lower than those of the control group（ P 〈0. 05）. 2SF-36 scores：After treatment,the scores of five dimensions including physical health,physical role,overall health,emotion role and mental health were markedly increased in the treatment group（ P〈 0. 05）,and the scores of the dimensions of physical health and mental health in the treatment group were higher than those in the control group（ P 〈0. 05）. Conclusion： ＂Rougan Decocted Extract＂combined with nucleoside antiviral drugs can significantly reduce the liver fibrosis indexes and improve the quality of life for patients with chronic hepatitis B.
