血清PSA、ALP与OC联合检测在前列腺癌骨转移中的临床意义 被引量:5

The clinical value of combination of PSA,ALP and OC in the bone metastasis of prostate cancer

摘要 目的探讨血清前列腺特异性抗原(PSA)、碱性磷酸酶(ALP)和骨钙素(OC)联合检测在前列腺癌骨转移中的诊断价值及临床意义。方法 81例前列腺癌患者根据核素骨显像诊断分为50例骨转移组和31例非骨转移组,对所有患者血清PSA、ALP和OC进行检测,分析2组患者PSA、ALP和OC的表达水平,比较和评价单个及指标联合检测时对疾病的诊断效能。结果血清PSA、ALP和OC在前列腺癌骨转移组样本中表达水平显著高于非骨转移组,差异具有统计学意义(P<0.05)。PSA、ALP和OC对前列腺癌骨转移的诊断参考值分别为40.60 ng/ml、125.26 U/L和3.96 ng/ml,检测灵敏度分别为78.4%、76.5%和68.6%,特异度分别为43.3%、46.7%和60.0%。PSA、ALP和OC的表达水平随着骨转移分级的增加而逐渐升高,差异具有统计学意义(P<0.05)。结论 PSA、ALP和OC检测能够用于前列腺癌骨转移早期诊断和预警,联合检测可提高其诊断效率。 Objective To explore the value and clinical significance of the combined detection of serum prostatic specific antigen( PSA),alkaline phosphatase( ALP) and osteocalcin( OC) in the bone metastases of prostate cancer. Methods 81 cases of prostate cancer patients were divided into bone metastases group with 50 cases and non-metastases group with 31 cases by radionuclide bone imaging. Serum levels of PSA,ALP and OC were detected in all patients. The expression levels of PSA,ALP and OC were analyzed,so as to compare and evaluate the diagnostic effectiveness of individual and combined detection for this disease. Results Serum PSA,ALP and OC levels of metastases group were higher than non-metastases group,and the differences were statistically significant( P〈0. 05). The diagnostic values of PSA,ALP and OC were 40. 60 ng / ml,125. 26 U / L and 3. 96 ng / ml,respectively,and the sensitivity were 78. 4%,76. 5% and 68. 6%,respectively,and the specificity were43. 3%,46. 7% and 60. 0%,respectively. The PSA,ALP and OC expression levels increased with the degrees of bone metastases,and the differences were statistically significant( P〈0. 05). Conclusion The combined detection of PSA,ALP and OC is useful for the early diagnosis and warning of prostate cancer,which can improve the diagnostic efficiency.
