摘要
目的:研究过敏性紫癜(henoch-schonleinpurpura,HSP)患儿外周血和肾组织中浆细胞样树突状细胞(pDC)含量及其临床价值。方法:选择过敏性紫癜合并肾损害的30例患儿纳入研究的HSPN组,选择过敏性紫癜不合并肾损害的30例患儿纳入研究的NHSPN组,健康志愿者30例纳入研究的对照组,检测3组患者外周血中pDC、Th2细胞及IL-4、IL-5、IL-10、IL-13含量以及肾脏组织中pDC、Th17细胞及IL-17、IL-21、IL-23含量。结果:(1)HSPN组和NHSPN组外周血中pDC细胞含量低于对照组且HSPN组外周血中pDC细胞含量降低更为明显;HSPN组和NHSPN组肾脏组织中CD304的含量高于对照组且HSPN组肾脏组织中CD304含量升高更为明显;均P<0.05。(2)HSPN组和NHSPN组外周血中Th2细胞及IL-4、IL-5、IL-10、IL-13含量高于对照组且HSPN组外周血中相关指标升高更明显;HSPN组肾脏组织中Th17细胞及IL-17、IL-21、IL-23含量高于NHSPN组;均P<0.05。(3)外周血中pDC的含量与Th2细胞含量及IL-4、IL-5、IL-10、IL-13含量呈负相关,肾脏组织中pDC的含量与Th17细胞含量及IL-17、IL-21、IL-23含量呈正相关。结论:pDC含量异常与过敏性紫癜的发病有关,外周血中pDC含量减少并且会造成Th2细胞功能增强,肾脏中pDC含量增多并且会造成Th17细胞功能增强。
Objective: To study the plasmacytoid dendritic cell (pDC) level in peripheral blood and renal tissue of chil- dren with henoeh-schonlein purpura and its clinical value. Methods.. A total of 30 henoch-schonlein purpura children with renal damage were enrolled in HSPN group, 30 cases of henoch-schonlein purpura children without renal damage were enrolled in NHSPN group, and 30 cases of healthy volunteers were enrolled in the control group. Then contents of pDC, Th2 cell, IL-4, IL-5, IL-10 and IL-13 in peripheral blood as well as contents of pDC, Th17 cell, IL-17, IL-21 and IL-23 in renal tissue of three groups were detected. Results: pDC contents in peripheral blood of HSPN group and NHSPN group were lower than those of control group and the decrease of pDC contents in peripheral blood of HSPN group was more significant. CD304 contents in renal tissue of HSPN group and NHSPN group were higher than those of control group and the increase of CD304 contents in renal tissue of HSPN group was more significant. Th2 cell as well as IL-4, IL-5, IL 10 and IL-13 contents in peripheral blood o{ HSPN group and NHSPN group were higher than those of control group and the increase of related indexes in peripheral blood o~ HSPN group was more significant Th17 cell as well as IL 17, IL-21 and IL 23 contents in kidney tissue of HSPN group were higher than those of NHSPN group. In peripheral blood, pDC content was negatively correlated with Th2 cell level as well as II74, IL-5, IL-10 and IL-13 contents, and in renal tissue, pDC content was positively correlated with Th17 cell level as well as IL-17, IL-21 and IL-23 contents. Conclusion: Abnormal pDC content correlates with the pathogenesis of henoch-schon- lein purpura, pDC content is decreased in peripheral blood, which results in enhancement of Th2 cell function, while pDC con- tent is increased in kidney, which result in enhancement of Th17 cell function.
