低剂量亚砷酸/维A酸方案治疗儿童骨髓增生异常综合征的临床观察 被引量:1

摘要 目的:观察低剂量亚砷酸/维A酸方案治疗儿童骨髓增生异常综合征(MDS)的临床疗效,为儿童MDS临床治疗规范化提供依据。方法:14例MDS患儿,其中6例为非初诊治疗病例,既往均曾长期间断应用Cs A等药物治疗。采用以低剂量ATO/ATRA为主的诱导缓解治疗方案,治疗时间至少达到1个疗程以上,进行疗效评估。结果:1)14例MDS患儿的近期CR 6例,m CR并HI 2例,HI 2例,SD 2例,PD2例,近期临床有效率为85.71%;且初诊患儿的临床疗效优于非初诊患儿,差异具有统计学意义(P<0.05);在12例有效患儿中,1个疗程即获得有效评价的病例占57.14%,起效中位时间为1个疗程。11例RCC患儿的近期临床疗效为81.82%,中危及以下患儿与高危及以上患儿临床疗效比较差异无统计学意义(P>0.05)。2)至随访截止时,14例患儿的中位PFS为18个月,中位OS为27.5个月。3)尚未发现本方案具有改变MDS患儿异常细胞核型的作用。4)在14例患儿的治疗中,未见明显的毒副反应,仍有10例患儿在维持治疗中。结论:低剂量亚砷酸/维A酸方案治疗儿童骨髓增生异常综合征具有良好效果。
