Twin-block矫治器治疗Ⅱ类骨型错牙合畸形患者的临床疗效分析

Clinical efficacy of Twin- block appliance in bone type of class Ⅱmaloclussion

目的观察Twin-block矫治器治疗Ⅱ类骨型错牙合畸形患者的临床效果。方法选择Ⅱ类骨型错牙合患者27例,应用Twin-block矫治器进行治疗,平均治疗时间为9.4个月,X线头影测量分析患者硬组织及上气道相关指标,对比治疗前后患者上述指标的变化情况。结果治疗前后SNA差异无统计学意义(P>0.05);治疗后SNB、LI-MP、Co-Po均增加,差异均有统计学意义(P<0.05),下颌骨位置前移,长度增加;治疗后ANB、UI-SN减小,差异有统计学意义(P<0.05);鼻咽段前后径宽度减小,差异无统计学意义(P>0.05);腭咽段、舌咽段、喉咽段前后径宽度增加,差异有统计学意义(P<0.05)。结论 Twin-block矫治器能有效促进Ⅱ类骨型错牙合患者下颌骨生长,增加上气道前后径宽度。

Objective To Observe the clinical effect of Twin- block appliance in treatment of bone type of class Ⅱ malocclusion.Methods 27 patients undergoing bone type of Ⅱ malocclusion were selected, and Twin- block therapy with 9.4 months average duration was applied.Hard tissue of patients＇ head and airway related indicators before and after the treatment were analyzed through cephalometric X ray, and then index changes were compared. Results one of index like SNA showed no significant difference before and after treatment（P〈0.05）; But other indexes like SNB, LI- MP, Co- Po were increased after treatment, and the differences were statistically significant（P〈0.05）. Furthermore, mandible position moved forward, and its length increased; ANB, UI- SN level decreased significantly after treatment（P〈0.05）; the width of the front and rear diameter of the nasopharynx segment decreased, but the difference was not statistically significant（P〈0.05）; the width of the front and rear diameter of pharyngeal, glossopharyngeal, hypopharynx and posterior segment increased, and the difference was statistically significant（P〈0.05）. Conclusion Twin- block appliance can effectively promotes mandible growth for classⅡ, bone type and error occlusal patients and increases the width of the front and rear diameter of upper airway.
