早产儿脑病的MR影像学表现及危险因素分析

MR Imaging Findings and Risk Factors Analysis of Premature Infants with Encephalopathy

目的:研究早产儿脑病的磁共振(MR)影像学表现及危险因素。方法:选取2014年2月-2015年2月在本院分娩的早产儿脑病患儿36例作为观察组,另选取同期早产无脑病患儿36例作为对照组,研究早产儿脑病患儿的MR影像学表现及危险因素。结果:经头颅MR检查,对照组36例早产儿结果正常,观察组36例患儿中,单纯的早产儿脑病患儿占91.7%,其他类型的脑损伤患儿占8.33%。早产儿脑病发病以非囊性为主占77.8%,囊性病例占13.9%,脑室周围白质存在损伤患儿占41.7%,脑叶白质发生病变患儿占58.3%。导致患儿发病的危险因素为阴道助产、用氧时间、多胎之大、体重、真菌败血症,与早产正常儿比较,差异具有统计学意义(P<0.05)。结论:对早产儿脑病患儿实施MR影像学检查,能够明确病因,找出危险因素。

Objective To study the preterm children encephalopathy MR imaging findings and risk factors. MethodsFebruary 2014- 2015 in February encephalopathy in premature children in our hospital 36 cases of childbirth as a research object, select another year without encephalopathy 36 cases of preterm control group of preterm children encephalopathy MR Imaging performance and risk factors. Results The cranial MR examination, the control group of 36 cases of premature children is normal, observation group of 36 cases of children, simple encephalopathy in premature children accounted for 91.7 % of children with other types of brain injury accounted for 8.33 %. The incidence of preterm children with non-cystic encephalopathy mainly accounted for 77.8 %, cystic cases accounted for 13.9 % of children with periventricular white matter injury in the presence of 41.7 % of children with cerebral white matter lesions accounted for 58.3%. Children with risk factors for the onset of vaginal delivery, oxygen supply, large multiple births, the weight, fungal sepsis, preterm delivery compared with normal children, the difference was statistically significant（P〈0.05）. Conclusion Preterm children encephalopathy implement MR imaging, can clear cause, identify risk factors, to facilitate the children can receive timely diagnosis and treatment.
