摘要
目的探讨受控衰减参数(CAPTM)在脂肪肝定量诊断中的应用价值。方法在健康体检中将超声诊断重度脂肪肝组120例,轻度脂肪肝组180例,正常肝脏组100例,均行CAPTM检测;以质子磁共振波普(1H MRS)为金标准,随机选取轻度脂肪肝组、重度脂肪肝组及正常组各10例与之对照。结果轻度脂肪肝组、重度脂肪肝组与正常组CAPTM值之间比较有显著差异(p<0.05)。CAPTM值越高,1H-MRS脂肪峰越高,与水峰的峰下面积比值越高,CAPTM与1H-MRS测定肝脏脂肪含量有高度一致性(r=0.884和r=0.711,p<0.01)。结论受控衰减参数(CAPTM)可以作为无创定量检测肝脏脂肪含量的方法,运用于健康体检人群并作为检后干预的评价指标。
Objective To investigate the application value of controlled attenuation parameter(CAPTM) in the quantitative diagnosis of fatty liver.Methods 120 cases diagnosed as severe fatty liver through ultrasound in physical examination group,180 cases diagnosed as mild fatty liver,and 100 cases diagnosed as normal group,were all performed CAPTM; 10 cases in each group were randomly selected and 10 cases in the normal group was as control group with proton magnetic resonance pop(1 H-MRS) as the gold standard.Results CAPTM value among mild fatty liver group,the severe fatty liver group compared with normal group was significant different(p 〈 0.05).The higher CAPTM value was,the higher 1H-MRS fat peak was,and the higher the water peak of the peak area ratio.Liver fat content had a high consistency between CAPTM and 1H MRS determination(r = 0.861 and r = 0.705,p 〈 0.01).Conclusion CAPTM can be used as a noninvasive quantitative detection of liver fat content,and applied in healthy check-up and been an evaluation indexes as inspection after intervention.
