摘要
目的通过分析心肌缺血患者血清缺血修饰白蛋白(IMA)和超敏肌钙蛋白T(hs-Tn T)水平的变化,研究联合检测IMA及hs-Tn T对心肌缺血的诊断价值。方法研究组选取2014年间在我院就诊具有胸痛、胸闷等心肌缺血症状患者50例,其中确诊心肌梗死患者35例,对照组为同时期在我院检查的健康人群50名。两组研究对象分别检测IMA、hs-Tn T、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平,对研究组患者的上述指标以每2小时1次的频率进行持续跟踪记录。通过两组研究对象指标数据的分析,研究IMA及hs-Tn T对心肌缺血诊断灵敏度、特异性,以及IMA、hs-Tn T水平变化对病情的评估作用。结果研究组IMA、hs-Tn T、CK、CK-MB水平明显高于对照组,P<0.05;IMA灵敏度为66.0%,特异性为45.5%,hs-Tn T灵敏度为26.0%,特异性为84.6%。联合检测IMA及hs-Tn T灵敏度为92.0%,特异性为76.1%。结论联合检测IMA及hs-Tn T水平在心肌缺血发展的整个阶段有利于早期诊断及病情的实时监控,具有重要的临床参考价值。
【Objective】By analyzing the changes of serum levels of serum ischemia modified albumin(IMA) and T(hs-Tn T) in patients with myocardial ischemia, the diagnostic value of serum ischemia modified albumin and T in serum was studied.【Methods】The research group selected 2014 cases in our hospital with chest pain, chest tightness and other myocardial ischemia symptoms in 50 cases, including 35 cases of acute myocardial infarction patients, the control group for the same period in our hospital 50 cases of healthy people. Two groups of subjects were tested for serum ischemia modified albumin(IMA), T(hs-Tn T), creatine kinase(CK), creatine kinase isoenzyme(CK-MB) levels, the study group of patients with the above indicators of the frequency of every 2 hours a continuous tracking record. The sensitivity and specificity of serum ischemia modified albumin(IMA) and T(hs-Tn T) on the diagnosis of myocardial ischemia were studied by the analysis of two groups of data.【Results】Serum levels of serum ischemia modified albumin(IMA), T(hs-Tn T), creatine kinase(CK), creatine kinase isoenzyme(CK-MB) were significantly higher than those in control group(P〈0.05), the sensitivity was 66%, specificity was 45.5%, sensitivity was 26%, specificity was 84.6%. The sensitivity of combined detection of serum ischemia modified albumin(IMA) and T(hs-Tn T) was 92%, and the specificity was 76.1%.【Conclusion】Combined serum ischemia modified albumin and serum troponin T levels are helpful for early diagnosis and disease monitoring in the whole stage of myocardial ischemia, and it has important clinical reference value.
