摘要 目的 了解我国结核病防治机构结核感染控制现状,为我国开展结核病感染控制工作提供参考.方法 2013年9月至2013年11月,采用目的抽样,对东部A省、中部B省和西部C省22家结防机构的门诊、病房、实验室的结核感染控制措施情况进行调查,其中东部A省有7家,中部B省有7家,西部C省有8家,实地调查结防机构的结核感染控制规章制度、分诊、宣传教育、通风、紫外线灯使用等情况,并填写《结核感染控制现场调查表》.调查表由一名课题组成员根据实地调查结果填写并由另一名调查员复核,收回的调查表的合格率达到100%.结果 22家结核病防治机构中,10家机构建立了结核感染控制的规章制度,设立独立挂号窗口的有6家,有独立候诊区的有5家,门诊通风量≥12ACH的机构有17家,实验室通风量≥12ACH的机构有16家,病房通风量≥12ACH的机构有5家.门诊紫外线灯照射强度≥70μW/cm2的机构有13家,实验室紫外线灯照射强度≥70 μW/cm2的机构有16家,病房紫外线灯照射强度≥70 μW/cm2的机构有5家.有8家机构的医务人员佩戴医用防护口罩.结论 22家结核病防治机构在结核感染控制方面整体较薄弱,需要加强结核感染控制的监控与评价,增加医疗卫生机构感染控制的人力、物力、财力投入,建立和完善感染控制的各项规章制度. Objective To investigate the status of tuberculosis (TB) infection control of TB prevention and control agencies, and to provide policy references for infection control in our country. Methods From September 2013 to November 2013, 22 agencies from A province of the Eastern (7 agencies), B province of the Central (7 agencies) and C province of the Western (8 agencies) were investigated. Infection control status of outpatient clinic, ward and laboratory in the TB prevention and control agencies were evaluated, including TB infection control regulations, triage, education, ventilation, the use of ultraviolet light. TB infection control questionnaires were filled in by one member of. research group and rechecked by the other. The qualification rate of the returned ques tionnaire was 100%. Results Twenty-two agencies participated in this survey, 10 agencies developed TB infection control regulations, 6 agencies set up independent registration windows, and 5 agencies established separate waiting area, 17 agencies' ventilation of outpatient surpassed 12ACH, 16 agencies' ventilation of laboratories surpassed 12ACH, and 5 agencies' ventilation of wards was more than 12ACH. Thirteen agencies' outpatient illuminate intensity of ultraviolet light was ≥70/μW/cm^2 , 16 agencies' laboratories illuminate intensity of ultraviolet light was ≥70/μW/cm^2 , and 5 agencies' wards illuminate intensity of ultraviolet light was≥70/μW/cm^2. Medical workers in 8 agencies wore respirator. Conclusion TB infection control in 22 TB prevention and control agencies is weak overall. We should strengthen the monitoring and evaluation of TB infection control, increase the manpower, material resources and financial resources on TB infection control in medical institutes, develop and improve the infection control rules and regulations.
