出院患者延续护理实施现状

Current status of transitional care for discharged patients: a review

延续护理是指将对患者的照护服务由医院延伸到社区或家庭,解决出院患者所面临的健康照护问题,以提高患者的生活质量。本文就出院患者延续护理的实施对象、实施方式、实施人员及我国出院患者延续护理存在的问题和建议进行综述,以为临床护理工作提供参考和借鉴。

Objective： The aim of transitional care is to ensure a safe transition of patients from hospital to community or home, and to solve problems of the discharged patient for improving the quality of life. This paper reviewed the objects, the implementation methods, the responsible personnel, the problem and the countermeasures of transitional care to the discharged patients.
