骨结核患者^(18)F-FDG PET/CT显像特征

摘要 目的探讨氟代脱氧葡萄糖正电子发射计算机断层显像(18F-FDG PET/CT)检查对骨结核诊断的价值。方法回顾性分析5例确诊为骨结核患者的18 F-FDG PET/CT图像,采用定性和半定量分析方法,结合葡萄糖摄取形态及CT影像学特点,综合判断病灶部位、累及范围及各部位病变性质。结果 5例骨结核可分为单发型、多发型和弥漫型,在PET/CT图像上特征性表现为环形或不均匀葡萄糖摄取增高、周围伴冷脓肿并出现钙化,或骨破坏区有明显沙砾样死骨。其中,弥漫型显示骨髓广泛性浸润(无明显骨破坏),与血行播散有关。结论骨结核18 F-FDG PET/CT显像有一定特点,对全身性结核的诊断和与恶性肿瘤鉴别诊断时有较大价值。 Objective To analyze the characteristics of bone tuberculosis on fluoro deoxidization glucose (18F‐FDG ) positronemission tomography (PET/CT ) .Methods The 18 F‐FDG PET/CT imaging was retrospectively analyzed in 5 cases with proven bone tuberculosis .The qualitative and semi‐quantitative analysises of standardized uptake value were used to determine the position and nature of the lesions by combining the morphology of glucose uptake and the characteristics of CT imaging .Results The bone tuberculosis of 5 cases was classified into three types of one lesion ,two or more lesions and diffuse type .On PET/CT images ,the lesions showed annular or uneven increasing glucose uptake ,around with cold abscess or obvious dead bone like gravel in lesions .The diffuse type showed blood flow‐related infiltration of bone marrow ,without bone destruction .Conclusion PET/CT imaging of bone tuberculosis shows some characteristics ,which has an important role in the diagnosis of systemic tuberculosis and identification of it from malignant tumors .
