扩张型心肌病心功能不全与室性心律失常的相关性分析 被引量:8

An analysis of correlation between cardiac insufficiency and ventricular arrhythmia in patients with dilated cardiomyopathy

摘要 目的分析扩张型心肌病(DCM)患者心功能不全与室性心律失常的相关性。方法DCM患者135例,行超声心动图、和24-h动态心电图监测,检测血浆B型利钠肽(BNP)。按纽约心脏病功能(NYHA)心功能分级和左室射血分数(LVEF)分组,比较室性心律失常的发生情况。结果DCM患者NYHA心功能分级与血浆BNP水平呈正相关(rs=0.851,P<0.05),与LVEF呈负相关(rs=-0.681,P<0.05)。随着心功能分级、BNP水平增高以及LVEF的降低,DCM患者严重室性心律失常的发生率增高(P<0.05)。结论 DCM患者心功能损害程度与严重室性心律失常的发生密切相关。联合应用血浆BNP和LVEF检测能预测DCM患者发生室性心律失常的风险。 Objective To analyze the correlation between cardiac insufficiency and ventricular arrhythmia in patients with dilated cardiomyopathy (DCM ) .Methods The examinations of echocardiography ,24‐h dynamic ECG and plasma B‐type natriuretic peptide(BNP) were performed in 135 patients with DCM ,who were grouped according to NYHA heart function classification .The correlation of the occurrence of ventricular arrhythmia and cardiac function was analyzed .Results The NYHA heart function classification in DCM patients was positively correlated with plasma level of BNP ( rs =0 .851 ,P〈0 .05 ) and negatively correlated with LVEF ( rs = -0 .681 ,P〈0 .05 ) .The incidence of severe ventricular arrhythmias in the patients with DCM was increased as the increases of the classification of cardiac function and plasma level of BNP and the decrease of left ventricular ejection fraction(LVEF)(P〈0 .05).Conclusion The cardiac dysfunction is closely related with severe ventricular arrhythmias in the patients with DCM .Combined examination of LVEF and plasma level of BNP is helpful in predicting the risk for the occurrence of ventricular arrhythmias in the patients w ith DCM .
