注射HCG后授精时间对IVF-ET妊娠结局的影响研究 被引量:5

摘要 目的 探讨注射人绒毛膜促性腺激素(HCG)后授精时间对体外受精-胚胎移植(IVF-ET)妊娠结局的影响.方法 回顾性分析河南省人民医院生殖医学中心2014年1月至2014年12月1301例IVF-ET新鲜周期. 以年龄,获卵数、受精率、2PN卵裂率、可利用胚胎率、临床妊娠率、胚胎种植率等为观察指标,按照注射HCG后不同授精时间分三组比较.A组为注射HCG后38 ~ 39 h,B组为注射HCG后39 ~ 40 h,C组为注射HCG后40~41 h.结果 A、B、C三组受精率、2PN卵裂率和可利用胚胎率比较差异均未见统计学意义(P>0.05),A、B组临床妊娠率和胚胎种植率分别与C组比较,差异均有统计学意义(P<0.05),A、B两组间比较时差异未见统计学意义 (P>0.05).结论 适合本中心IVF的授精时间是在注射HCG后40 ~41 h,可提高临床妊娠率和胚胎种植率,从而获得更好的临床结局. Objective To investigate the effect of the insemination time after the injection of HCG on the pregnancy outcome of IVF-ET.Methods One thousand three hundred and One IVF-ET fresh cycles from January 2014 to December 2014 were retrospectively analyzed.Each cycle was analyzed by age, number of eggs, fertilization rate, 2PN cleavage rate, available embryo rate, clinical pregnancy rate and implantation rate.According to the different insemination time after injection of HCG divided into three groups.Group A(38 ~39 hours after injection of HCG), group B(39 ~40 hours after injection of HCG) and groupC(40 ~41 hours after injection of HCG).Results There was no significant difference in fertilization rate, 2 PN cleavage rate and available embryo rate among the three groups (P 〉 0.05).Significantly higher clinical pregnancy rate and implantation rate were observed in group C(70.30% 53.97%), compared with group A(65.13% 47.97%) and group B(63.53% 47.23%) ,P 〈 0.05.Conclusions The appropriate insemination time is 40 ~ 41 hours after the injection of HCG, can improve the clinical pregnancy rate and implantation rate, so as to obtain better clinical outcome.
