摘要
目的探讨超声对肝脏特异性感染的诊断价值,以辅助临床治疗。方法随机选取2013年5月-2014年5月收治的20例肝脏特异性感染患者,应用彩超、二维超声检查,部分加用超声造影检查和病理结果进行对照,数据采用SPSS 17.0软件进行统计分析。结果彩超及常规二维超声结果,肝脏真菌感染8例患者、肝脏结核感染9例、肝脏梅毒感染3例,分别占38.10%、42.86%、14.29%;12例患者行超声造影检查,造影剂注入之后,动脉相、门静脉相、延迟相结节均无增强发生,表现为"充填缺损",仅3例肝脏真菌感染,观察结节周边,有扫描增强,20例患者,在超声引导下活检,均取材成功,其中超声引导下穿刺活检证实15例,肝脏结核感染9例、肝脏真菌感染6例。结论肝脏特异性感染采用二维超声和彩超诊断具有一定价值,超声引导下穿刺活检,可对大部分肝脏特异性感染病变检查,并做出明确诊断。
OBJECTIVE To investigate the function of ultrasonography in the diagnosis of liver specific infections so as to provide help in the clinical treatment. METHODS Totally 20 patients with liver specific infections admitted from May 2013 to May 2014 were selected randomly. Color ultrasound testing and conventional two-dimensional ultrasonic testing were applied. And unltrasound contrast was added to part of the patients. The results were com- pared and SPSS 17.0 was explored to make statistical analysis. RESULTS The results of Color ultrasound testing and conventional two-dimensional ultrasonic testing showed that eight cases suffered liver fungal infections and 9 cases were with liver tuberculosis, 3 cases got syphilis infections in the liver, accounting for 38.10%, 42.86%, and 14.29% respectively. Totally 12 cases underwent CEUS. After Contrast injection, no enhancement of artery nodular, portal vein nodular and delayed nodular appeared and all patients manifested as "filling defects". Only three cases got fungal infections of the liver with enhanced peripheral nodules, There were 20 cases successfully sampled with biopsy under ultrasound guidance. Among them, 15 cases were confirmed: 9 cases with liver tuberculosis infections and 6 cases with fungal infections of the liver. CONCLUSION Two-dimensional ultrasound and color Doppler ultrasound were proved to be effective in the diagnosis of liver specificity infections. Ultrasoundguided biopsy can give definitive diagnosis of most liver specificity infections.
