胃癌术后化疗患者医院感染病原菌与耐药性分析

Pathogens causing nosocomial infections in patients received chemotherapy after gastric cancer surgeries and the drug resistance

目的研究分析胃癌术后化疗患者医院感染特征,探讨病原菌的构成比及耐药性,为临床合理使用抗菌药物提供参考依据。方法随机选取2011年12月-2013年12月医院接收救治的200例胃癌术后化疗患者,对其临床数据展开回顾性分析,采集患者标本进行病原学检测及药敏试验,数据采用SPSS 12.0软件进行统计分析。结果 200例胃癌术后化疗患者发生医院感染45例、47例次,感染率22.5%、例次感染率23.5%;感染部位以肺部感染为主,共34例次占72.3%;共培养出62株病原菌,以革兰阴性菌为主,共37株占59.7%,其中铜绿假单胞菌占32.3%;铜绿假单胞菌和肺炎克雷伯菌对庆大霉素、哌拉西林、头孢呋辛耐药率较高,均>78.0%。结论胃癌术后化疗患者医院感染中常见病原菌以铜绿假单胞菌、金黄色葡萄球菌为多见,医务人员要合理运用抗菌药物控制病原菌的耐药性以及病原菌的传播。

OBJECTIVE To study the features of nosocomial infections in patients received chemotherapy after gastric cancer surgeries and to discuss the distribution and drug resistance of pathogens so as to provide references fo rclinial use of antibiotics. METHODS Totally 200 patients received chemotherapy after gastric cancer surgeries from Dec. 2011 to Dec. 2013 were selected randomly. The clinical data of them were analyzed retrospectively. The samples from patients were collected to conduct etiological test and plored to make statistical analysis. RESULTS Totally 45 patients and 47 sensitivity test. SPSS 12.0 was ex cases go t infected and the infection rate was 22.5% and the case infection rate was 23.5%. Totally 34 cases got infected in lungs, where proved to be the major, infection sites, accounting for 72.3 %. A total of 62 strains of pathogens were cultured, mainly gram-negative bacteria with 37 strains, holding 59.7%. Among them, Pseudomonas aeruginosa took up 32.3%. Pseudomonas aeruginosa and Klebsiella pneumoniae were highly resistant gentamicin, piperacillin, and cefuroxime with resistance rates all over 78. 0%. CONCLUSION The common pathogens causing infections in patients received chemotherapy after gastric cancer surgeries are mainly P. aeruginosa and Staphylococcus aureus. Medical staff should rationally use antibiotics to control the drug resistance of pathogens and cut the spread of them.
