摘要
目的分析我院INSURE策略治疗早产儿呼吸窘迫综合征(RDS)失败的发生率和相关危险因素。方法回顾性调查2010年1月到2014年12月入住我院新生儿科并接受过INSURE治疗的RDS早产儿的临床资料,比较成功组和失败组围产期危险因素的差异,根据logistic回归分析得出INSURE策略治疗早产儿RDS失败的危险因素。结果共109例早产儿纳入本研究,我院INSURE策略治疗早产儿RDS失败率为32.1%,失败组出生体重明显低于成功组(P<0.05),动脉导管未闭(PDA)发生率较成功组显著增加(P<0.05);失败组的PaO2、PaO2/FiO2均明显低于成功组(P<0.05),PaCO2较成功组明显增高(P<0.05)。根据logistic回归分析,出生体重<1000g、小于胎龄儿、动脉血PaCO2(mm Hg)>50mm Hg、PaO2/FiO2<190、X线分级重度是INSURE治疗失败的独立危险因素。失败组氧疗时间、死亡率明显高于成功组(P<0.05)。结论 INSURE策略治疗早产儿RDS失败的危险因素是出生体重<1000g、是否小于胎龄儿、动脉血PaCO2(mm Hg)>50mm Hg、PaO2/FiO2<190、X线分级重度是INSURE策略治疗早产儿RDS失败的危险因素。
Objective To study the risk factors for the failure of the In Sure method in preterm infants with respiratory distress syndrome(RDS). Methods One hundred and nine preterm infants with RDS treated with In Sure method were enrolled. These infants were categorized into two groups:INSURE success and INSURE failure. The differences in basic information were compared between the two groups,and logistic regression analysis was used to identify the risk factors for In Sure failure. Results The failure rate of the In Sure method was 32.1%. The failure group were much lower in the birth weight than the success group(P〈0.05). The incidence of patent ductus arteriosus in the failure group was significantly higher than in the success group(P〈0.05). PaO2,PaO2/FiO2 in the failure group were significantly lower than in the success group(P〈0.05). PaCO2 in the failure group was much higher than in the success group(P〈0.05). Further logistic regression analysis showed that birth weight〈 1000g,small for gestational age,PaCO2〉50mm Hg、PaO2/FiO2〈190 and severe radiological grade,were the independmend risk factors for INSURE failure. Furthermore,the duration of oxygen therapy and the mortality in the failure group were much longer and higher than in the success group(P〈0.05). Conclusion Low birth weight,small for gestational age,PaCO2〉50mm Hg、PaO2/FiO2〈190 and severe radiological grade are the risk factors for the failure of the INSURE method in preterm infants.
