右美托咪定复合喷他佐辛用于老年患者髋关节置换术后镇痛的临床观察被引量：7

摘要目的观察右美托咪定复合喷他佐辛用于老年髋关节置换术后自控静脉镇痛(PCIA)的效果。方法选择ASAⅠ-Ⅱ级行髋关节置换术患者40例,随机分为A组、B组各20例。A组给予喷他佐辛40ug/(kg·h)+托烷司琼5mg,B组给予喷他佐辛40μg/(kg·h)+右美托咪定1μg/(kg·d)+托烷司琼5mg。设置两组术后PCIA持续量2ml/h,单次自控输注量2ml,锁定时间20min,总量100ml。观察比较2组在术后2h(T1)、6h(T2)、12h(T3)、24h(T4)、48h(T5)的视觉模拟评分(VAS)评分、镇静评分(Ramsay评分)及各时段自控镇痛的按压次数,记录不良反应发生情况。结果两组术后各点的VAS评分和RSS评分无明显差异(P>0.05),A组在T1-T2、T2-T3、T3-T4三个时段的自控按压次数高于B组(P<0.05),A组术后恶心呕吐和寒战发生率显著高于B组(P<0.05)。结论右美托咪定用于老年患者髋关节置换术术后镇痛可减少喷他佐辛用量,并且可以减少术后恶心呕吐和寒战的发生率。
