补中益气汤加味联合西药治疗糜烂型口腔扁平苔藓疗效观察

Observation of Buzhong Yiqi Decoction combined with western medicine in erosive oral lichen planus

目的观察西药常规治疗联合补中益气汤加味治疗糜烂型口腔扁平苔藓的临床疗效。方法将符合纳入标准的80例糜烂型口腔扁平苔藓患者随机分为治疗组与对照组各40例,2组均给予常规西药治疗,治疗组加用中药补中益气汤加味治疗,疗程均为4周,比较2组临床疗效及治疗前后CD4+、CD8+、CD4+/CD8+变化。结果治疗组总有效率明显高于对照组(P<0.05),而复发率明显低于对照组(P<0.05);2组治疗1,2,4周时VAS评分均较治疗前明显降低(P均<0.05),且治疗组较对照组降低更加明显(P均<0.05);治疗4周后,治疗组中医证候积分及CD4+、CD8+水平均较治疗前、同期对照组明显降低(P均<0.05),CD4+/CD8+均较治疗前、同期对照组明显增高(P均<0.05)。结论西药常规治疗联合补中益气汤加味治疗糜烂型口腔扁平苔藓疗效好,可明显缓解疼痛,减轻伴随症状,其作用机制可能与纠正T淋巴细胞平衡紊乱有关。

Objective It is to observe the clinical efficacy of conventional treatment of western medicine combined with Buzhong Yiqi Decoction in the treatment of erosive oral lichen planus. Methods 80 patients with erosive oral lichen planus were randomly divided into treatment group and control group with 40 cases in each. All cases were given conventional western medicine treatment,and combined with Chinese medicine Buzhong Yiqi Decoction in the treatment group. After treatment for4 weeks,the clinical curative effect were compared,and the levels of CD4~＋,CD8~＋and CD4~＋/ CD8~＋were detected before and after treatment in both groups. Results The total effective rate in the treatment group was significantly higher than that in the control group（ P 〈 0. 05）,but the recurrence rate was significantly lower than that of the control group（ P 〈 0. 05）; the VAS score in two groups were significantly lower after treatment for 1,2,and 4 weeks（ all P 〈 0. 05）,and the treatment group was significantly lower than that in control group（ all P 〈 0. 05）; after treatment for 4 weeks,the TCM syndrome integral and the levels of CD4~＋and CD8~＋were significantly lower than before or control group（ all P 〈 0. 05）,but the value of CD4~＋/ CD8~＋was significantly higher（ P 〈 0. 05）. Conclusion The routine treatment of western medicine combined with Buzhong Yiqi Decoction in the treatment of erosive oral lichen planus with good curative effect,can effectively relieve the pain and reduce the accompanying symptoms. Its mechanism may be related to rectify the disorder of T lymphocyte homeostasis.
