三维彩色血管能量成像检查联合人附睾蛋白4和糖链多肽抗原125用于卵巢癌早期诊断的临床价值

摘要 目的探讨三维彩色血管能量成像(3D—CPA)联合人附睾蛋白4(HFA)和糖链多肽抗原125(CA125)检查用于卵巢癌早期诊断中的价值。方法150例经二维彩色多普勒诊断为卵巢混合性包块的患者,术前进行血清HE4和CA125检查,并联合3D—CPA进行定性分析,术后病理检查确诊包块性质。分析以上各方法的灵敏度、特异度等。结果HE4对卵巢癌诊断的灵敏度及特异度均高于CA125。HE4与CAl25联合检测的灵敏度及特异度均高于单独HE4或CA125检测,差异有统计学意义(P〈0.05)。HFA+CA125+3D—CPA可以提高卵巢癌早期诊断的特异度、阴性预测值和准确率(P〈0.05),但敏感度、阳性预测值没有明显改善(P〉0.05)。结论HEA、CA125、3D—CPA联合检查可以显著提高卵巢癌早期检出的特异度和准确度。 Objective To evaluate the application of HE4, CA125 combined with 3 - dimensional color power angiographie (3D -CPA) in the diagnosis of early ovarian cancer. Methods A total of 150 patients who were diagnosed with ovarian mixed mass by two -dimensional ultrasound preliminary diagnosis were enrolled in the current study. Their levels of HE4 and CA125 were determined before surgery, which were used in combination with 3D - CPA for qualitative analysis. The masses were pathologically examined after surgery. The sensitivity and specificity of these methods were an- alyzed. Results HE4 has better sensitivity and specificity than CA125 for diagnosis of ovarian cancer. The combination of HE4 and CA125 showed better sensitivity and specificity than HE4 or CA125 alone (P 〈 0.05 ). 3D -CPA when used in combination with HE4 and CA125 could improve the specificity, negative predictive value and accuracy (P 〈 0.05 ), with no obvious changes in sensitivity and positive predictive value (P 〉 0.05). Conclusion HE4, CA125 combined with 3D - CPA can remarkably improve the specificity and accuracy of ovarian cancer detection at an early stage.
