摘要
目的 阻塞性睡眠呼吸暂停低通气综合征 ( OSAHS) 患者应用高频超声对颈动脉结构进行研究, 为OSAHS的诊治提供依据. 方法 66例OSAHS患者, 均为男性, 年龄27~72, 平均年龄 (51.9 ±10.2) 岁; 体质量指数(BMI) 为 (29.1 ±3.4) kg/m2. 对照组66例 (健康无OSAHS志愿者), 均为男性, 年龄28~73, 平均年龄 (52.1 ± 9.8) 岁; BMI为 (25.9 ±3.2) kg/m2. 用超声对颈动脉进行二维超声检查, 测量颈动脉内一中膜厚度 ( IMT)、 收缩期内径 (Ds)、 舒张期内径 (Dd), 并测量肱动脉收缩压 (SBP)、 舒张压 (DBP) 和脉压 (PP), 计算颈动脉紧张度(AS)、 扩张性 (AD) 和僵硬度 (ASt), 评价颈动脉顺应性. 结果 OSAHS 组 IMT、 SBP、 DBP 较对照组增大( P0.05); PP、 Ds、 Dd 2组间差异无统计学意义 ( P〉0.05); OSAHS组颈动脉AS、 AD较对照组减小, ASt较对照组增大 ( P〈0.05). 结论 高频超声能够反映颈动脉结构和功能变化, 为OSAHS患者诊治的临床评价提供颈动脉顺应性变化的监测数据, 具有临床应用价值.
Objective Sleep apnea hypopnea syndrome( OSAHS) patients with high-frequency ultrasound to study the structure of the carotid artery,provide the basis for the diagnosis and treatment of OSAHS.Methods 66 cases of OSAHS pa-tients were male,aged 27-72(mean age 51.9 ±10.2)years;body mass index(BMI)was(29.1 ±3.4)kg /m2.66 cases in the con-trol group(healthy volunteers without OSAHS),were male,aged 28-73(mean age 52.1 ±9.8)years;BMI was(25.9 ±3.2)kg /m2 .With two-dimensional ultrasound for carotid ultrasound,measuring a carotid artery thickness( IMT) ,internal systolic diameter (Ds),diastolic diameter(Dd),and brachial systolic blood pressure(SBP),diastolic blood pressure(DBP)and pulse pressure (PP),calculated carotid tension(AS),expansion(AD)and stiffness(ASt),evaluation of carotid arterial compliance.Results OSAHS group IMT,SBP,DBP increased compared with the control group(P〉0.05);PP,Ds,Dd between the two groups showed no significant difference(P〈0.05); OSAHS carotid AS,AD is reduced compared with the control group,ASt increased compared with the control group(P<0.05).Conclusion High frequency ultrasound to reflect changes in the carotid artery structure and function,carotid artery compliance monitoring data available for the clinical evaluation of changes in the diagnosis and treatmentof OSAHS patients have clinical value.
