全方位优质护理干预对肝硬化患者治疗依从性和生活质量的影响

Effect of comprehensive quality nursing intervention on treatment compliance and life quality of patients with liver cirrhosis

目的探讨肝硬化患者采取全方位优质护理干预对肝功能和生活质量的影响,为临床护理提供指导意义。方法选取2013年1月-2014年5月来我院门诊及消化内科住院的肝硬化患者156例作为研究对象,随机分为对照组和观察组：对照组76例,采用常规消化内科护理;观察组80例,在常规消化内科护理基础上给予全方位优质护理干预,记录患者护理期间治疗依从性状况,采用生活质量评定问卷SF-36评价患者生活质量,同时调查患者对护理干预的满意度状况。结果两组患者在休息和活动、合理饮食、烟酒控制、正确服药、定期复查的依从性上均存在显著差异（Z=-3.362、-3.887、-3.450、-2.931、-3.197,P〈0.01）;两组患者在护理干预前生活质量指标无差异无统计学意义（P〉0.05）,对照组和观察组患者干预护理前后生活质量指标存在显著差异（P〈0.01）,两组患者间干预护理后生活质量指标存在显著差异（P〈0.01）;两组患者在干预护理后满意度上存在显著差异（P〈0.01）。结论采用全方位优质护理干预能改善患者生存质量和治疗依从性,患者护理满意度高,值得推广。

Objective To explore the effect of comprehensive quality nursing intervention on liver function and life quality of patients with liver cirrhosis, and to provide guidances for clinical nursing. Methods A total of 156 patients with liver cirrhosis who were hospitalized in the Outpatient Department and the Department of Gastroenterology in our hospital from January 2013 to May 2014 were selected as research subjects. They were randomly assigned to the control group and the observation group. 76 patients in the control group were given regular nursing in the department of gastroenterology, and 80 patients in the observation group were further given comprehensive quality nursing intervention on the basis of nursing in the department of gastroenterology. Treatment compliance of patients were recorded during nursing, life quality assessment questionnaire SF-36 was applied to evaluate patients＇ life quality, and at the same time patients＇ satisfaction rate towards nursing intervention was investigated. Results There were significant differences between the two groups of patients in terms of their compliance with resting and activities, proper diet, control of smoking and alcohol, correct drug administration, and regular re-examination（Z=-3.362,-3.887,-3.450,-2.931,-3.197, P〈0.01）;there was no difference between the two groups of patients in terms of life quality index before nursing intervention（P〈0.05）, there was significant difference of life quality index before and after nursing intervention in the control group and the observation group（P〈0.01）, and there was significant difference between the two groups of patients in terms of life quality index after nursing intervention（P〈0.01）; there was significant difference between the two groups of patients in terms of satisfaction rate after nursing intervention（P〈0.01）. Conclusion Application of comprehensive quality nursing intervention is able to improve patients＇ life quality and treatment compliance with high patients＇ satisfaction rate towards nursing, which is worthy of promotion.
