无创呼吸机治疗急性左心衰的临床研究

Curative effect research of noninvasive ventilator on acute left heart failure

目的 观察无创呼吸机治疗急性左心衰的临床疗效。方法 将我院2012年7月~2013年7月收治的68例急性左心衰患者随机分为观察组和对照组,各34例。观察组予以无创呼吸机治疗;对照组予以常规抢救。治疗后,比较两组患者的疗程及生命体征变化情况,评定其体力、焦虑情绪变化及随访1年生活质量。结果 与对照组有效率76.47%比较,观察组有效率94.12%升高明显,差异有统计学意义（P〈0.05）。与对照组比较,观察组患者的血压（SBP、DBP）、HR、HAMA评分总分及各因子分均降低明显,而血氧饱和度、体力稳定率升高明显,差异有统计学意义（P〈0.05）。观察组患者随访6、12个月时的PF、RP、RE得分,与对照组比较均升高明显,差异有统计学意义（P〈0.05,P〈0.01）。结论 无创呼吸机可有效改善急性左心衰患者的生命体征,提高临床疗效及患者体力,改善其生活质量,具有较好的临床推广价值。

Objective To observe the curative effect of noninvasive ventilator on acute left hears failure. Methods 68 patients with acute left hears failure in our hospital from July 2012 to July 2013 were selected and divided into observation group and control group,with 34 cases in each group.The patients in observation group were treated with noninvasive ventilator therapy,the patients in control group were treated with traditional therapy.Arterial clinical efficacy and vital signs between two groups were compared,the quality of life for 1 year of following up,physical changes and HAMA was assessed respectively. Results The total effective rate in observation group （94.12%） was better than that ：in control group（76.47%）,the difference was statistically significant（P〈0.05）.SBP,DBP,HR,HAMA total scores;mental factor scores and physical factor scores in observation group after the treatment was lower than in control group respeetively（P〈0.05）. Oxygen saturation was higher than that in control group,the differences was statistically significant （P〈0.05）.Two groups of patients were alive,and no lost to follow-up,compared with control group,the observation group patients were followed up for six months,1 year of PF,RP,RE scores was increased obviously respectively,statistically significant difference （P〈 0.05;P〈0.01）. Conclusion Noninvasive ventilator in the treatment of acute left heart failure can significantly improve the patients＇ arterial vital signs,enhance the treatment efficiency and physical strength,improve the quality of life, and have better clinical value.
