曲安奈德联合利多卡因局部注射配合功能锻炼治疗膝关节创伤性滑膜炎的效果观察

Effect observation of trimcinolone combined with local injection of lido-caine and functional exercise in the treatment of traumatic synovitis of knee Joints

目的探讨曲安奈德联合利多卡因局部注射配合功能锻炼治疗膝关节创伤性滑膜炎的疗效。方法 将2014年1月~2015年2月我院收治的106例膝关节创伤性滑膜炎患者按随机数字表法分为观察和对照组,每组53例。观察组患者采用曲安奈德联合利多卡因局部注射配合功能锻炼的治疗方案,对照组采用生理盐水联合庆大霉素关节腔冲洗后注射治疗方案,观察两组治疗效果。结果 观察组患者总有效率为92.4%,对照组总有效率为73.6%,组间比较,差异有统计学意义（P〈0.05）;观察组治疗后及治疗后3个月VAS评分分别为（3.31±1.08）、（1.51±0.66）分,对照组治疗后及治疗后3个月VAS评分分别为（4.13±1.06）、（2.56±0.97）分,观察组优于对照组（P〈0.05）。两组治疗后日常功能及肿胀均较治疗前有改善,差异有统计学意义（P〈0.05）;观察组肿胀程度优于对照组,差异有统计学意义（P〈0.05）。结论 曲安奈德联合利多卡因局部注射配合功能锻炼治疗膝关节创伤性滑膜炎是一种行之有效的治疗方案,效果满意。

Objective To discuss the effect of triamcinolone combined with local injection of lidocaine and functional exercise in the treatment of traumatic synovitis of knee joints. Methods 106 patients with traumatic synovitis who treated in our hospital from January 2014 to February 2015 were divided into observational group and control group with each group having 53 patients by use of randomized number table.Patients in the observational group were treated with triamcinolone combined with local injection of lidocaine and functional exercise while patients in the control group were injected with normal saline after articular cavity irrigation by gentamicin.Effect of treatment between two groups was compared. Results Total effect rate of patients in the observational group and the control group was 92.5% and 73.6% respectively（P〈0.05）.VAS score after treatment and 3-month after treatment was respectively （3.31±1.08） points and （1.51±0.66） points in the observational group while the VAS score was respectively （4.13±1.06） points and （2.56± 0.97） points in the control group.The results of the observational group was superior to the results of the control group （P〈0.05）.Normal functions and swelling was improved respectively after treatment between two groups,with statistical difference （P〈0.05）.As for swelling degree,the observational group was superior to the control group and the difference was statistically significant （P〈0.05）. Conclusion Triamcinolone combined with local injection of lidocaine and functional exercise in the treatment of traumatic synovitis of knee joints is an effective treatment regimen and its effect is satisfactory.
