摘要
目的 探讨组织弥散定量技术中应变均值(MEAN)评估不同病程桥本病(HD)的价值。方法 收集2014年3月-2015年8月经昆明医科大学第五附属医院临床确诊的HD患者108例,根据实验室甲状腺功能检查结果分为HD甲功正常组(n=31)、HD亚临床甲减组(n=41)、HD临床甲减组(n=36),另选50例健康者作为对照组。入选者均采用组织弥散定量分析,测量甲状腺组织感兴趣区域的MEAN,使用单因素方差分析及LSD-t检验进行组间和组内差异性比较,使用Spearman法分析其与HD病程的相关性。结果 病例组MEAN为68.84±18.41;对照组MEAN为121.58±20.86,差异有统计学意义(P〈0.05);甲功正常组、亚临床甲减组、临床甲减组的MEAN分别为89.92±6.47、70.25±5.23、44.56±10.03,各组间MEAN差异有统计学意义(P〈0.05)。MEAN与不同病程HD的相关系数r=-0.906(P〈0.01)。结论 MEAN有望对诊断HD、评估其病程进展提供新的客观指标。
Objective To explore the value of MEAN in tissue dispersion quantitative technology to assess Hashimoto disease (HD) with the different course of disease different. Methods 108 patients with HI) clinically diagnosed in the Fifth Hospital Affiliated to Kunming Medical University from March 2014 to August 2015 wele collected.Patients were divided into HD thyroid function normal group (n=31),HD subclinical hypothyroidism group (n=41) and HD clinical hypothyroidismgroup (n=36) according to the laboratory examination result of thyroid function.50 health people were selected as control group.MEAN of region of interest (ROI) in thyroid tissue was measured.The diffel:ence of inter-group and intra-group was compared by using one-way anova analysis and LSD-t test.Spearman method was used to analyze the relationship between it and course of diseae in HD. Results MEAN in case group was 68.84±18.41,which was 121.58±20.86 in control group,and the difference was statistical significance (P〈0.05).The MEAN in thyroid function normal group,HD subclinical hypothyroidism group and HD clinical hypothyroidism group was 89.92±6.47,70.25±5.23 and 44.56±10.03 respectively,and the difference was statistical significance (P〈0.05). The correlation coefficient between MEAN and different HD course was -0.906 (P〈0.01). Conclusion MEAN will to provide mew objective index for diagnosis of HD and assessing the progress of state of an illness.
