摘要
目的分析脑梗死患者血清氧化低密度脂蛋白(ox-LDL)、C反应蛋白(CRP)、基质金属蛋白酶-9(MMP-9)与颈动脉弹性指标的相关性。方法选取2015年1—12月动脉粥样硬化性脑梗死(ACI)患者110例(ACI组)、腔隙性脑梗死(LACI)患者90例(LACI组)和健康人群100例(健康对照组),检测血清ox-LDL、CRP和MMP-9水平,颈动脉压力应变弹性系数(Ep)、僵硬度(β)、顺应性(AC)、增大指数(AI)及脉搏波传导速度(PWVβ),分析上述指标之间的相关性。结果(1)与健康对照组比较,LACI组Ep、β、AI、PWVβ值增高,MMP-9、ox-LDL、CRP水平增高,AC降低,差异有统计学意义(P<0.01);与LACI组比较,ACI组Ep、β、AI、PWVβ值增高,MMP-9、ox-LDL、CRP水平增高,AC降低,差异有统计学意义(P<0.01)。而各组间TC、TG、FPG均无明显差异(P>0.05)。(2)脑梗死患者血清ox-LDL、CRP、MMPs-9水平与β、PWVβ呈正相关(r=0.346、0.305,0.686、0.835,0.664、0.712,P均<0.01)。结论脑梗死患者血清ox-LDL、CRP和MMP-9与颈动脉β、PWVβ有明显相关性,三者均是早期发现动脉弹性病变的敏感指标。
Objective To analyze the correlation between cerebral infarction patient's serum oxidized low density lipoprotein(ox-LDL),C-reactive protein(CRP),matrix metalloproteinase 9(MMP-9) and carotid artery elasticity index.Methods From 2015 January to December,110 cases of atherosclerotic cerebral infarction(ACI) patients were enrolled as ACI group,90 cases of lacunar cerebral infarction(LACI) patients were enrolled as LACI group,and 100 cases healthy people enrolled as healthy control group,detected level of serum ox-LDL,CRP,MMP-9,carotid artery pressure strain elastic coefficient(Ep),stiffness(β),compliance(AC),increase index(AI) and pulse wave velocity(PWVβ),and analyzed the correlation between these indices.Results(1) compared with the healthy control group,LACI group's Ep,B,AI,PWVβvalues were increased,MMP-9,ox-LDL,CRP levels were increased,AC were decreased,the difference were statistically significant(P〈0.01);compared with the LACI group,the ACI group's Ep,β,AI,PWVB values were increased,MMP-9,ox-LDL,CRP levels were increased,AC was decreased,the difference were statistically significant(P〈0.01).There were no significant difference of TC,TG,FPG among different groups(P〈0.05).(2) Cerebral infarction paitents' serum level of ox-LDL,CRP,MMP-9.B,PWVβ were positively correlated(r =0.346,r =0.305,r =0.686,r =0.835,r =0.664,r =0.712,all P〈0.01).Conclusion Cerebral infarction patients' ox-LDL,CRP,MMP-9,β,PWV β were significantly correlated,these indices can be used as a sensitive indicator to early detect of arterial elasticity related diseaes.
